Employee was lifting the end of a pipe with a chain attached to an excavator. Two fingers got caught in the chain.

Employee slipped when entering front-end loader and hurt his forearm. Injury was not reportable initially. On 11/16/2006, we were notified by his doctor that he will need surgery.
