Contract EE was helping replace a belt. Slack gave way knocked him off balance he tried to get his balance his knee gave out. He said that his knee has given out a few times before prior to employment, from past injury. He went to medical facility and was put in a brace and crutches. Conditions contributing are: sudden movement, soft ground, history of knees going out, weight.

While moving some material from behind the Primary Crusher he hurt his right arm. He said it bothered him that evening and he thought it would feel better the next day. He notified me the following day that it still bothered him.

The operator said that he backed to close to the edge of the bench and it gave way causing the dozer to go off the the left side first.

EE was cutting a piece of flashing for a wiper on S-2 conveyor. The razor knife slipped and stabbed him in the left knee on the inside of the knee cap.
