Wile roof bolting face area, a loose rock fell between rib and 1st installed roof bolt. Rock was 3' long X 2' wide X 1' thick. Rock hit canopy and operator moved into path of rock. Struck by rock in back of head and ankle.

Employee was tramming a feeder at x-cut 25 between beltline & haulage. Loose rock fell out & fell on the employee pinning his shoulder & left leg.

A roof fall occurred in the main west return. The fall was 50' long, 18' wide and 7' high. The fall was between xc 18 and 19 xc and not in the intersections.

A rock fell off a corner in #3 entry outby face. Employee had moved out of the way of a shuttle car going to miner when a rock came out of the top. Hit him on the shoulder, arm & right foot.

A roof fall occurred in the 2nd main north entries at #12 xc, #1 to #2 entry. The fall measured 20 feet long, 17 feet wide and 8 feet thick.

Miner operator had backed continuous miner out to intersection to get a rock off miner. After rock had been removed, continuous miner was trammed into face to load a shuttle car. After shuttle car came in & got loaded, s/c operator started tramming toward feeder & as he got to rock in intersection, he hit it with car, flipping it into operator's cab. Size of rock 5'x 4'x 8"
