The employee and operator were Positioning a steel suction pipe for installation on the dredge. The pipe was about 3 feet off the ground and one end was hitting the excavator cab. The operator tried to help by reaching out the cab front window. EE's jack hooked the down lever, causing the pipe to strike the laborer's left leg

Showing all 5Employee was tightening a pump gasket with an open wrench. The wrench slipped off and struck EE in the mouth as EE was pulling the wrench towards self to tighten. Employee was taken to the Emergency Room and the cut required 2 stitches. Employee returned to work after the visit to the Emergency room. The employee was returned to work with no restrictions.
Loaderman was pulling on a 5' pipe wrench attached on a booster pump shaft when the wrench slipped off and the handle struck him on the head and he required 5 stiches. He was oked to return to work.
Truck driver was walking to get weigh ticket when he lost his footing in loose stone and fell bruising his head,shoulder,knee and leg.Was checked at doctors office and xrays showed no problems.
Employee was dismantling 10' pipe when the top of his left hand rubbed across a burr in pipe and cut his hand. It required 7 stiches
