A screen panel was being replaced in a triple-deck screen by two mechanics. One mechanic was working inside the screen loosening the fasteners with a battery impact and ratchet wrench. While ratcheting, the mechanic felt and heard a pop in his right shoulder. A medical evaluation revealed a strained/sprained tendon, and the mechanic was placed on restricted duty for healing.

A mechanic was removing the hub, remaining blades, bearings, shaft, and belt sheave from a 72-in fan by hand. During the process of physically attempting to lift the assembly out of the fan, the mechanic strained his lumbar muscles.

Coarse material was loaded into a Cat 772 by a Cat 988, which caused the truck and operator to be shocked very hard. The truck operator was leaning forward on the steering wheel, and his seat pressure had been lower for road comfort. The shifting truck caused him to be slammed into the seat which hurt his upper back and made it hard for him to breath (thoracic strain).

A stuck pin was attempting to be dislodged during a rotary scaling head installation on an excavator/scaler. The former pin was being held to drive the new pin back out by an operator. When struck, the pin bounced back out, and struck the operator's right hand.

Employee was loading shot when stood up right knee popped out of place. Employee reported this is not the first time his knee went out, happened in previous employment. MRI done on 7/10/12 showed weak Patella, employee was given a brace and set up for physical therapy. Since incident employee has been able to work within his underground miner 1 classification.

A scrap return idler was physically being thrown into a roll-off dumpster by a miner. The idler rolled off the edge of the dumpster and struck another miner on the top of his right foot. The miner was wearing metatarsal shoes so the injury was limited bruising, swelling, and pain.

On 11/10/11, the mechanic was welding overhead on a roof bolter drill slide hose protector. Hot slag rolled onto his left hand and burned his middle finger. The mechanic applied first aid treatment at the time of the incident, but did not report the incident as he felt that it was a minor incident. The burn became infected/abscessed and required medical treatment on 11/22/11.

A conveyor transfer had plugged and spilled material around the tail section. The area was being cleaned up by the production crew utilizing manual labor and a mini skid-steer. A 3-ft section slip-lock PVC piping was attempted to be pulled from an elbow to provide add'l working room. Unfortunately, the pipe slip-joint connection was stronger than perceived by the miner.

The plant operator was carrying two 5-gal buckets of lube oil to a cone crusher lube system. The operator shared that he was not paying full attention to his travelway when he stepped in a vehicle rut and twisted his right knee. The operator completed his shift, requested medical treatment the following day. An MRI and follow up reviled a sprained ACL.

EE our Lab technician was removing sampling trays from the Gilson shaker. She did not have a firm grip on the tray and it came back towards the top of her right knee. The tray hit this area resulting in swelling and pain. She tried ice it but it was still swollen and it was hard to walk on.

During the preshift the foreman found a roof fall in the # 9 heading from 2+00 to 4+00. The fall measured 200' X 40' X 8'. There were 6' resin bolts used and the fall occurred at 8'.

HVAC contractor was applying caulk to the outer exhaust fan shield from a ladder. The ladder went out from under EE and his right index finger caught the sheet metal flashing around the fan shield. First aid was applied at the sign in room and his fellow employee drove EE to the Hospital where he received ten stitches and a referral to a hand physician.

Miner was working underground around station 45+00 (return slope), removing pics with saddle bar and 4 lb hammer, from ATM 105 mining equipment. Miner missed bar and hit left thumb with hammer.

The employee was pulling a water line with a Kawaski mule underground into crosscut #4. While operating, the employee looked over his left shoulder with his right arm through the steering wheel. The employee ran over an unseen rock in his travel path. The obstruction caused the steering wheel to turn sharply, pulling his right arm.

EE was setting up the robotic shotcrete machine in the travelway entry to apply shotcrete to the roof and ribs. After laying out the accelerator line to the area of the robot arm she was dazed by a blow to the back of her neck. She called out to co-worker who was about 70 feet away. They found her on one knee holding her hand on her head with horizontal abrasions on her neck.
