Employee was not wearing a company issued hard hat. Employee hit his head on viper frame. Employee was taken to the emergency room and received sutures.
A belt clip was torn on a conveyor belt. While cutting the belt, the miner mis-used the tool using to cut the belt clip and cut his pinkie and ring finger on his left hand. Operator error was determined to be the cause of the injury. No lost time occured from the injury.
