The injured employee and another person were removing a guard from the continuous miner. They lost grip of the guard, resulting in the injured employee's hand getting caught between the guard and the mine floor.
Cable was hanging from the top, something caused the cable to come lose. The cable struck the injured employee by hitting him in the chest and putting him on his back.
The injured person was reaching for a crib block. A coworker of the injured person was tossing crib blocks. As the crib blocks where being tossed one of the crib blocks struck the hand of the injured person which has resulted in a laceration requiring stitches.

The belt was in the process of coming to a complete stop, when the injured person reached toward the drive roller pulling him into the moving belt, causing him to strike an I beam and guarding around the belt head.

Injured man broke cut off valve for deluge fire spray. He turned the water off on the 6 inch line, when he bled the pressure off the line, it blew his hand into a pizza pan which was part of the bearing surface for roof support and recieved a cut to his finger.

There was no ventilation interruption nor did this fall affect the passage of men. The area will be cribbed and dangered off.
Miner was working on section power center raised up hooking ear lobe on a rope cable hanger tearing ear lobe.
