Employee had been using an 8 lb sledge hammer to open and close valves in precipitation unit. Employee reported pain and burning in left elbow.

Employee was drilling bottom level tap on 1-5 flash tank. Employee said drill packing failed and steam and hot liquor sprayed the employees right arm and abdomen. Employee received second degree burns.

Caustic leaked into boot through small leak.

Employee was dragging a workstand to where it was needed. There was a build-up of hydrate on the pad that created resistance to moving the stand. Employee felt pain in his lower and upper back.

Employee was closing a screen box and pinched his left index finger between a bolt and a slot for the bolt.

Employee was cleaning old gasket material from a set of flanges being held apart with a wedge. The wedge slipped out, allowing the flanges to move and caught the employees right ring finger between the flanges. Employee was given prescription pain medication, making it a recordable.

Employee had just installed a new drain valve and was attempting to drain 5-1 mud block. After rodding out the drain nozzle, the drain valve came loose and he was sprayed with hot caustic slurry.

Employee was removing bolts from a flange. He was operating an air operated impact tool with right hand while holding back-up wrench with left hand. The socket bounced off the nut and twisted his forearm.

Employee was clearing suction line to 45-7 sump pump by beating on it with an 8# sledge. After several minutes employee turned to get an air lance when the suction line cleared and blew liquor and hydrate onto the employee. His hard hat and goggles were knocked off alowing caustic liquor to contact his face and left eye.

Empolyee stepped through a hole in the grating and fell onto a discharge pipe and injured his left rib area.

Employee was using hot condensate hose down sacle from the tail pulley area of 48 ball mill. The hose was equipped with a hose end valve. The employee closed the hose end valve and the hose ruptured spraying the employee's right wrist and right forearm.

Employee was in the process of hosing down pad on north side of #33-18 sump. Employee turned his hips to his left and felt a pain in lower back and left leg.

Employee was moving a large piece of scale to a hole in the floor of the north side of #4 washer when the scale slid against his leg causing pain to his left ankle.

Employee slipped in mud and hit the inside of his right knee on the deck pad eye on the inside of the power cable reel.

Employee was cleaning a scooter and slipped and fell on the wet pad. Employee landed on left hip, causing pain around the waist and into right groin. This incident became recordable on 11/13/08 due to referral to physical therapy and prescription meds.

Using impact gun to remove flange bolts and the impact gun slipped off the nut and resulted in shoulder strain.

Employee was using an 8# sledge hammer to operate a valve. After srtiking the valve handle employee felt pain in his right shoulder. The employee was given perscription pain meds on 11/12/08.

Employee was attempting to close an angle valve using an 8# sledge hammer. After repeated blows with the hammer, the valve freed-up and the valve handle swung around and struck the employee on the back of his left hand.

On 10/16/08, employee was using an impact gun with his right hand in the trigger when he began experiencing pain to right wrist. Employee received prescription medication on 11/18/2008.

Employee was using a water lance to unplug a feed chute to 3-1 weigh belt. Employee was holding the lance pointing it upward when a piece of scale fell striking the lance. Employee states this caused the lance to jerk up while he was still holding it and caused a strain to both shoulders.(shoulder surgery refer due to day off by 220090630041)

Employee was leaning over tool box to remove a chop saw and felt a pain in his middle back.

Using an air impact gun to loosen flange bolts.

Employee pinched thumb between a rivet gun tool bit and a heat exchanger head while pushing bouls out that had become bound from process scale.

Employee was in a squatting position and as he shifted from a squat to a standing position, employee lost his balance and twisted while coming upright. That is when he felt a pain in his back.

Employee was squatting down to go under a steam line when his left foot slipped causing a "pop" in his right hip and right lower back.

Employee strained his lower back while swinging 8 lb sledge to open a valve.

Employee was walking down stairs at #3 zone, slipped and noticed pain in his right knee.

Employee was checking for tube leaks on bottom of a vertical heat exchanger. As employee bent to look at bottom tube sheet, his face shield came into contact with flange bolts and knocked it backwards. Just then, a tube leaked caustic slurry into employee's left eye.

Employee was using a hollow drill stem to adjust a turnbuckle to make adjustment to vertical drilling rig. He threw the drill stem instead of walking it around, it came around and struck him behind the left ear. The resulting laceration required three staples to close the wound.

EE was washing hydrate with a caustic/water mixture using a 1.5" hose. While adjusting the height of the hose tamer, some hydrate & caustic splashed back onto his faceshield & thru the vent holes in his googles. This resulted in a minor chemical burn near his eye & some irritation to his right eye. EE lost no time & has been moved from the area to prevent furthur irritation.

EE was working in an area containing hydrate when his left leg began to sink in a soft spot, while trying to get his leg free, some hydrate got under his slicker suit resulting in a minor caustic burn to the calf area where his rubber boots stop. EE was transferred to another area due to the inability to wear rubber boots until the area is no longer tender. No lost time.

Employee was rodding out a drain valve. When rodding was finished, he turned and started walking away. The drain valve began spewing hot slurry and employee was sprayed on the right hand, left elbow, waist and back of neck causing second degree burns.

Hit left knee with valve handle.

Employee was climbing stairs and tripped. He broke the fall by catching himself with his right hand. He later felt pain in his right wrist. The medical diagnosis is wrist fracture.

Sustained Right Ankle injury while walking under #3 digester train.

Employee opened gate valve, then raised his right arm to wave at another employee and cut his arm on a burr on the end of valve stem.

Swinging 8# sledge hammer to operate a valve.

Employee was filling conveyor drive hydraulic reservior using a 5 gallon bucket. Employee lifted the bucket and turned to his right and foot slipped on oil. The employee reported a sharp pain to his left knee.

Employee was walking on a slick surface, slipped and caught himself without falling. Employee then felt pain in the lower abdomen. The condition was diagnosed as an inguinal hernia.

EE WAS WORKING AT TOPSIDE CHARGE HEADER, WELDING PIPE. SLAG FELL INTO WELDING SLEEVE OF [R] ARM CAUSING 2ND DEGREE BURN TO [R] FOREARM. NO LOST TIME OR RESTRICTED DUTY.

EE WAS WORKING ON #10 PRESS, FELT OVERHEATED & EXHAUSTED. ECT WAS CALLED AND EE WAS TAKEN TO MEDICAL. IV FLUIDS WERE GIVEN BY PLANT MEDICAL TECHNICIAN.

EE walked through standing hot condensate and could not see the ground. As he crossed a drainage ditch, he stepped on a rock or piece of scale and twisted his left ankle. This caused him to fall into the hot condensate. EE attempted to get up and fell a second time. EE received burns and blisters on both shins.

Employee was shutting of a circulation pump. As employee walked around to valve it out, he tripped on grating falling to right side. Employee tried to break fall with hand causing laceration to middle knuckle. X-ray shows fracture to right middle finger.

While replacing an angle valve, employee attempted to push valve into place by twisting and lifting it, strained his abdomen resulting in a hernia.

Employee was opening valve on #2 rod mill with a sledge hammer. Employee slipped on oil spot on floor and felt a sharp pain in lower back.

Employee was removing a short angle iron support when it unexpectedly shifted and caused a one inch laceration to his left 5th finger requiring stitches.

EE was closing a block valve. Valve was almost closed and EE gave it jerk. The wrench slipped off, hitting his index finger between and the valve. Finger was fractured and required sutures and splint.
