Employee was cleaning old gasket material from a set of flanges being held apart with a wedge. The wedge slipped out, allowing the flanges to move and caught the employees right ring finger between the flanges. Employee was given prescription pain medication, making it a recordable.

Employee had just installed a new drain valve and was attempting to drain 5-1 mud block. After rodding out the drain nozzle, the drain valve came loose and he was sprayed with hot caustic slurry.

Employee was removing bolts from a flange. He was operating an air operated impact tool with right hand while holding back-up wrench with left hand. The socket bounced off the nut and twisted his forearm.
Employee was clearing suction line to 45-7 sump pump by beating on it with an 8# sledge. After several minutes employee turned to get an air lance when the suction line cleared and blew liquor and hydrate onto the employee. His hard hat and goggles were knocked off alowing caustic liquor to contact his face and left eye.

Employee was using hot condensate hose down sacle from the tail pulley area of 48 ball mill. The hose was equipped with a hose end valve. The employee closed the hose end valve and the hose ruptured spraying the employee's right wrist and right forearm.

EE was washing hydrate with a caustic/water mixture using a 1.5" hose. While adjusting the height of the hose tamer, some hydrate & caustic splashed back onto his faceshield & thru the vent holes in his googles. This resulted in a minor chemical burn near his eye & some irritation to his right eye. EE lost no time & has been moved from the area to prevent furthur irritation.

EE was working in an area containing hydrate when his left leg began to sink in a soft spot, while trying to get his leg free, some hydrate got under his slicker suit resulting in a minor caustic burn to the calf area where his rubber boots stop. EE was transferred to another area due to the inability to wear rubber boots until the area is no longer tender. No lost time.

EE walked through standing hot condensate and could not see the ground. As he crossed a drainage ditch, he stepped on a rock or piece of scale and twisted his left ankle. This caused him to fall into the hot condensate. EE attempted to get up and fell a second time. EE received burns and blisters on both shins.
