Injury was not reported at the time of the incident or while on SWN property. Injured employee reported to his dispatcher after leaving the sand mine that he was getting down from his trailer after opening his lids and slipped on the trailer ladder causing him to fall to the ground. Driver was hooked to SRL at time of incident.

Employee was using cutting torch to remove a damaged piece of 1/8" angle from the roof support on generator containment. Employee began removing by cutting the right side where it was connected to the structure. when the cut was complete, stored energy caused the angle to move hitting the employee in the lip causing a cut.

Employee was tightening bolt on dredge and felt pain in back. Employee had previous back condition, prior to employment.

Employee was stepping over a raised 10" pipe on the dredge. The employee did not see that there was a small (3") pipe laying on the ground on the other side of the 10" pipe. When he moved his left leg over the 10" pipe his foot came down on the smaller pipe causing him to roll his ankle.

Employee was sweeping the control room at the end of his shift using a broom when metal handle broke approximately 2' from the end of the handle cutting his middle finger.

Employee got foreign material in his left eye

Employee was on top of trac trailer opening hatches and checking for debris. Truck began move forward, when this happened employee's retractable lanyard (which was attached to life line system) locked into place causing employee to be jerked backwards landing on top of trailer.

Injured was performing maintenance on a 14' screw conveyor and when employee turned the shaft manually his (R) pinky finger became pinched between the belt and the shive pulley. His fingernail became detached. Injured was taken to the Emergency Room and treated. The fingernail was reattached with two stitches to keep the nail in place while healing.

Employee was removing a pump from a 500 gallon above ground storage tank. As he was removing the pump diesel drained from the suction tube and splashed on the employees face which ran into his eyes.

Employee reported to his supervisor approximately 1 1/2 hours after his shift ended that his back was hurting and wasn't sure what he did to hurt it. He went to the doctor and reported that he was having back spasms.

Employee strained his left knee while attempting to get a piece of equipment unstuck and stepped down into a shallow rut.

Employee was shoveling sand and reported that he hurt his back while twisting to load the sand into a wheel burrow.

Employee was shoveling spilled material from overhead conveyor systems into a wheel barrow and he claimed to have strained his lower back while dumping the wheel barrow.

Employee reported to have severe pain in his lower back and was taken to the doctor and later diagnosed with kidney stones. He was off for several weeks before we learned that he was referred to a specialist for a work related injury with having a compressed disc/nerve.

Employee was helping another employee to exit a surge hopper that was taken out of service for maintenance. During the process, the employee that was doing the lifting strained his lower back.

Injured stated incident happened on 2/1/2010 but did not report the incident until 2/2/2010. At first it was believed to be a spider bite but now the medical doctor is stating it is a staff infection and is treating employee for a staff infection.

Injured was assisting in lifting a portable stair with a total of five people. The lift was eight inches. When the group made the lift, injured felt pain in his lower back. He was taken to Baptist Medical Clinic in Little Rock, AR.

INJURED WAS MOVING A POWERED STACKER CONVEYOR HOLDING THE START BUTTON IN THE ON POSTION AS THE CONVEYOR MOVED WHEN HIS RIGHT THUMB SLIPPED OFF AND HE APPLIED ALL HIS FORCE RESULTING IN A SPRAIN/STRAIN TO HIS RIGHT THUMB.

INJURED WAS WALKING ACROSS A FIELD OF MUD WHEN HE STEPPED INTO SOME SOFT MUD APPROXIMATELY 8" DEEP, TURNED TO LOOK BEHIND HIMSELF, TWISTED HIS RIGHT KNEE.
