Showing all 19Employee was lifting up a piece of slate with a chisel to put a shim under it. Employee was holding the shim incorrectly with EE's thumb on top, and EE's other fingers on the bottom. The rock slipped out from the chisel and it fell on top of the block which then cut off part of the thumb and cut EE's middle finger.
Employee was trimming slate chips using Trimmer #1. She had a chip in the trimmer when the blade got stuck on the left side of the machine in the rock. She tried pushing the rock in, and the blade let loose and cut her finger. She had her fingers on top of the slate which is an unsafe act. Finger are always to be underneath the slate chip.
Employee was shoveling debris under conveyor belt in Roofing mill. He picked up a load that was too heavy, and the shovel tipped to the right and the load fell out of the shovel. He felt immediate pain in his right arm. Employee reported the pain the next day. He could not twist his arm or his wrist.
Employee was cleaning up conveyor belts in Roofing Mill. He pushed a rock slab aside and the rock fell over on to the employees hand.
Employee was operating Euclid R40 and had to do some maintenance. He climbed down ladder, then as he was climbing back up ladder, he got to the top rung and his foot slipped. He grabbed himself with the grab bar with his right hand.
Employee was hammering a pin into the track on a bulldozer. The pin split and a piece broke off and penetrated his leather boot and entered his ankle.
Employee was putting oil in loader when he lost his grip on the hold bar of his loader. He was able to grab on to the loader without falling, but in the process he dislocated his right forefinger.
Employee was picking up a rock to split it. The rock split in two as he was lifting it, and it fell back on to the conveyor rollers. The rock then had a smaller piece break off and flew up and hit the employee's face. He received a laceration to his right eyelid. All required PPE's were being used.
Employee was on Secondary Crusher belt when a co-worker started up the plant without following procedures. A second co-worker noticed that the plant had been started up with the employee on the belt and immediately yelled to stop the plant. The employee suffered minor injuries, which resulted in time off from work.
Employee operating the splitter. He was moving it and the splitter hit an uneven area and dropped and rocked and shook employee around in his seat.
Employee and coworker were removing a piece of cut rock from the saw table. Employee had his right hand on the rock and when a coworker moved the cut piece of rock, it slammed into his right hand pinning it between the cut rock and the main rock.
Employee was moving a rock on a saw table to prepare it for cutting. He was using a hammer to move the rock, as was a co-worker. The rock moved and the employee came down with his hammer and hit his hand instead of the rock.
Employee was manually spinning the crushing drum at the Crushing Plant when his foot slipped and got hung between the crushing drum and wear plates. His right foot may be fractured, but is definitely sprained and has soft tissue injury.
EE was splitting a rock on the ground with a hammer and hit his left hand.
EE was pushing/lifting a heavy stone. Legs went out from under him. He did a split and felt severe pain.
Employee was operating the R40 haul truck when the right front tire struck a rock. The steering wheel spun around sharply and right hand got caught up in the steering wheel.
EE was using a metal bar to dislodge a rock from the crusher and the bar came back and hit him in the right hand.
Employee was injured while placing a 2x4 on the ground to elevate a rock. The rock came off of the table. When it landed the 2x4 shot out from underneath and hit the employee's right ankle. Today 6/23/09, this was reported to me because the employee wanted to seak medical attention. He finished work on friday and also worked monday 6/22/09 all day
