Mechanism beam hung on motor base. Used slate bar to pry beam away from motor base. Beam came loose and caught right hand between motor base and beam, causing a laceration on the ring finger.

EE, at the beginning of the shift was climbing the mounting ladder on the 789C Rock truck and as EE went to grab the grab rail on the front of the cab, missed the rail with hand and as a result fell backwards and landed on back & right hip.

Employee finished splicing 39-B belt and began training the belt for proper alignment. As they were adjusting the structure, they pulled on a piece of top structure and they slipped on the wet mine floor. When they slipped and fell, they twisted their back and felt pain in mid to upper back.

EE is the section electrician and was resetting a side breaker on the right-side continuous miner. As EE was leaving the are a section of the rib fell and caught EE's feet, with the left foot being the primary complaint.

Employee was attempting to cut a nylon zip tie with a utility knife. Access to the zip tie was very limited, employee's hand slipped on the knife and employee cut right index finger, the wound required stitches (6) to close.

EE was carrying a can of welding rods, when EE fell, causing the palm of EE's hand to strike the sharp edge of the can and EE suffered a laceration to EE's left hand. The injury required stitches to close the wound.

EE was installing a rib bolt when EE heard a "pop" in the roof at the ATRS, as EE turned to move away EE felt pain in right knee. EE worked full duty until 9/11/20 when EE needed medical attention. EE was placed on restricted duty at that time.

EE was helping to move a MRS that was stuck in bad mine floor conditions. EE, along with several crew members, were assembling and spacing the metal slats and clevises to connect the C/M to the MRS. While handling and positioning the slats and clevises EE suffered a strained back. EE could not remember the exact time of the event.

Employee was hooking a chain to a chute that was being removed. After hooking the chain, employee stepped down off the chute and a jagged piece of metal punctured left boot in the area near the arch of left foot, causing a laceration of the foot.

EE was part of a crew assigned to install a hard-bar pin in the D9T dozer on the mud dump parking area. The pin was aligned and placed to be driven in place with a sledgehammer. When the pin was struck with a hammer a fragment of the pin fractured off and struck EE in the face. EE received a laceration that required stitches.

Employee was climbing up the access steps of the 777C rock truck to clean the mirrors. EE lost footing and fell backwards to the ground. Employee landed on back and began experiencing pain in the neck, back, and hips as a result of the fall.

EE was dismounting the 777B rock truck to refuel. When EE stepped from the bottom step to the ground, (approximately 30"), left ankle rolled over when foot contacted the ground. EE began to experience pain and swelling in the ankle.

EE was adjusting a curtain so EE could scoop/clean the #5 Left x-Cut. While doing so EE heard a "pop" and when EE turned a piece of rock fell from the roof, between the rib bolt and the rib. EE was struck on the lower left side and on top of EE's hip bone.

EE was standing beside the scoop hanging the 12" waterline from a rib bolt with a chain when the chain slipped out of the eye of the plate causing the waterline to fall and strike EE in the top of the left ankle.

Employee was walking to Pre-Shift a pump, when EE stepped on a loose rock, slipped and fell to the ground. This resulted in a fracture to the tibia just above the ankle. This was diagnosed on March 7th, when EE visited the ER.

Worker was aligning two pieces of pipe on a pump in order to bolt them together. One piece of pipe fell; and as a result, smashed and cut open the pinky finger on worker's right hand. Received outpatient treatment: 13 stitches to close wound and support brace applied to finger.

Employee was working on re installing a rub rail on a 14/15 CM and was using a pry bar. As employee moved the rail with the bar it slipped and struck employee's left wrist. On 2/13/2020 employee followed up with a Doctor and was diagnosed with an unspecified fracture of the lower end of left radius.

Employee, (Continuous Miner Operator) was found, by the shuttle car operator, pinned between the right rib and the right side of the continuous miner. Employee was backing the miner out of the #2 heading after a 10' cut. During this process employee was securing slack in the miner cable by placing a pull strap on a bit and bit lug on the cutter drum.

An unintentional roof fall occurred in the No. 8 entry on the North Mains section. The fall occurred as the Continuous Miner operator was backing the miner out of the last lift. Material fell from the lift just completed and extended into the entry, pulling bolts and catching the miner head. No Injuries were suffered from this event.

An unintentional roof fall has occurred just outby spad 7022, measuring 15' X 8'. In addition, roof conditions have deteriorated and affected travel in the Primary escapeway that runs through the Mains and 5A sub-mains panel.

Employee was handling the roof bolter cable when the machine moved forward the cable caught EE's ring finger of EE's left hand. EE suffered a laceration that required 1 stitch to close and a finger nail was pulled off. Employee was wearing gloves at the time of the injury.

An unintentional roof fall occurred in the 3 Left cross- cut of the North Mains unit. The fall occurred in the cut and travelled into the intersection. Area was approximately 16 to 18 feet wide, 30 feet long and approximately 10 to 12 feet high at the highest point. Area involved was two crosscuts inby spad 2129.

EE was using a sledgehammer to drive the coupling latches down on the highwall miner push beams. As EE attempted to swing the hammer to secure one of the latches, EE felt left shoulder popped. EE experienced severe pain in left shoulder forcing EE to stop work assignment.

Employee was riding on a rubber tired mantrip when a piece of belt struck the rescuer box, dislodging it and pinning employee's right foot and ankle. Employee later went to Dr.'s office with swelling and was diagnosed with a contusion and restricted employee's activities.

Using a lifting jack to re-rail a track duster, when the jack bar slipped and pinched EE's finger middle finger on EE's right hand between the bar and a rail. This resulted in a laceration that required stitches to close the wound.

EE was injured after completing installation of a row of roof bolts. After completing the row, EE moved back to the center of the bolting machine. After moving back, EE inadvertently placed EE's left hand on a frame plate near the ATRS, resulting in hand placement in the pinch point of the ATRS. EE suffered soft tissue injuries only.
