Showing all 5Field Mechanic using impact wrench to secure wheel to a 774 Caterpillar off-highway truck. The wheel dropped as the mechanic was positioning it causing EE's left middle index finger to be pinched between the wheel and the rim.
EE stated was getting off from equipment AT-14 facing the equipment when EE stepped down on a rock which caused EE to slip and lose footing causing EE to fall and land on their back. Handrails on equipment were in good condition and steps were dry and free of any damage. EE has been instructed to use a knee brace and crutches.
Employee was pouring concrete and got some in EE's boot, untimely causing chemical burn and abrasion. Employee did not report until 11-05-21. Employee went to the doctor and was ultimately released to full duty on 12-07-21
Loader operator, was stacking material on a stockpile, while traveling up a stock pile with a load, had to down shift, also pulled on the bucket control acidently engaged the auto lift control. Loader was at a slight angle to the right, operator right front tire drove into depresion, caused it to lay onto its right side, was wearing seatbelt, no injuries.
