An electrician working in the area of West A feeder belt passed a barricade when walking down the catwalk, and fell through to the ground below. The area had previously been barricaded off due to unstable catwalk platforms that needed to be replaced/fixed.
The employee was operating a loader in which they were having trouble reaching the foot pedals, which irritated their back. The employee was then placed on a mechanical scaler in which the operation there of, irritated their back even more. The employee notified their supervisor and was taken to seek medical attention.
Miner was walking around cleaning the drill with a pressure sprayer wand, when miner fell, in doing so it caused the wand to rotate back to miner's face. The water stream cut a small laceration above miner's eye that required two stitches to close.

During removal the wash unit tipped from the telehandler forks and guard on washer unit caught the mechanics glove cutting EE's right middle finger and scraping stomach. Mechanic was taken to the hospital and received 4 stitches in EE's right middle finger.
Employee was using torch cutting bolts off and burned through glove in turn burning left index and middle fingers.

A pump sitting on a trailer was moved by RWS contractors by a telehandler, without notification to Nyrstar. RWS hitched the trailer to their telehandler. RWS pulled the trailer up a ramp. The trailer hitch broke. An individual RWS EE traveling on foot behind the trailer was struck
Operator was traveling down a roadway in a haul truck when struck a pot hole. Operator felt discomfort in back and the incident cause lower back pain and spasms. Incident did not become restricted or reportable till 5/4/2021 after further evaluation from healthcare provider.
