Employee climbed over a barricade and travelled in a non-designated area. Employee was stepping over a gap between a retaining wall and a primary jaw crusher and did not step far enough. The height from the top of the wall to the floor is approx. 11 ft. The employee stated bifocal sunglasses made employee misjudge the distance employee needed to step.

Showing all 3EE was in shop dismounting a chute from a crusher. He was using an impact wrench to take off a nut from a carriage bolt, while holding head with his right hand. He pushed the trigger on the impact wrench and the locking part of bolt popped out of place. This cause the head to spin fast and cut through glove and into finger.
Injury Was inspecting the scalping screen and under jaw belt wiper. He was steppeing from the screen to the belt, slipped, and striking the end of the production 2 belt with hes back and then falling to the ground. He fractured his T2 - T5 vertebra and a right tibial contusion and knee sprain.
