John was checking on employees on the dredge in the pit. EE exited EE's truck to walk to the berm of the pit for a visual inspection. Upon exiting, EE stepped into a hole where a tree had been removed. EE fell, but EE's leg/ankle stayed straight in the hole, hearing a pop.

Right foreman came in contact with conveyor causing laceration requiring sutures.
Employee was shoveling sand at the sand plant and had soreness in employee's back. The next morning employee could hardly move with the pain from the muscles in chest and back.
Employee was walking to his excavator and slipped and fell on the ice. Fractured left ankle from fall.
