Miners were cutting a boom hole to set a belt drive. The injured employee was helping the miner operator when a piece of steel came back, broke the employee's safety glasses and struck him in the right eye.

Victim was pulling drill stlls out of the roof, while pulling them out the drill still fell out of hole that was drilled bounced off the drill head and struck the victim in the mouth breaking 3 teeth and caused a small cut on his lower lip.

Employee was repairing the discharge line for a surface pump. While cutting the discharge line off of a coupler, his knife slipped and he stabbed his left thumb.

Victim and scoop operator were moving a bucket liner of bolter supplies. Bucket liner started to come out of bucket, so victim flagged scoop operator and got in bucket liner to hook up a chain. The scoop operator started to travel inby and as he went over a roll in the bottom, the scoop bucket raised up and crushed the victim against the mine roof.

While building overcasts underground, employee lifted a steel beam and pulled two tendons in his shoulder. Employee has had bursitis in his shoulder for a long time and the lifting caused it to become inflamed. Employee continued to work until 5/2/12 when he went to the doctor.

Employee was loading belt structure onto a skid on the surface. While holding a top roller, the employee turned and slipped on some ice and twisted his left knee. The employee continued to work until 1/30/2011 when he went to the doctor.

He slipped and fell on ice, which had stones sticking out of the ice. When he put down his hand to break his fall, the stones cut his right hand.

Employee was descending the ladder on the right side of the loader, his gloved right hand slipped loose from the ladder handrail, causing him to fall backward to the ground. He landed on his back and had the wind knocked out of him. His back was hurting and he reported the accident to the job foreman. The temperature was 15 degrees and there was frost on the equipment.
