Threads in water manifold were worn and let PPE nipple come out of manifold and sprayed water in employee's eye and face. Employee seen doctor and was discharged. Said eye was irritated.

Operator was bolting top, went to let drill head down and worked wrong lever, swung canopy into left hand, pinching hand between drill steel and canopy.
