EE & another EE was putting the trailer that contained the welder into the shop. The pickup that the trailer was hooked to died and would not start. EE got a floor jack to move the trailer on into the shop. The jack hit a crack between the shop floor and the landing the trailer dropped off the jack. Allowing the handle of the floor jack to come up and strike ee in the face.

He unloaded seven pallets of conveyor parts, while unloading the eighth pallet, weighing 415 lb total, two conveyor parts (return under rollers) slid off the top and fell into the cab of the skid steer striking the employee's left hand.
