End loader was pulling up Highwall Miner cable slack. The cable got tight and started sliding toward Employee. EE tried to jump out of the way but caught EE's left ankle knocking EE to the ground
The injured person was traveling down the haul road belonging to River Ridge P-1, 15-19895, to go home when the brakes failed and they jumped out of the vehicle causing injury to their right shoulder and laceration to their head.
Employee was working on Truck AC when hose and clamp on high pressure side blew off striking EE in the forehead.
Employee was standing on rear step of 980H endloader doing electrical work when step broke causing EE to fall injuring EE's left leg.
Employee pulled right side rub rail off miner and dropped it on left foot.
Employee was climbing over Continuous miner to put oil in gear case when EE bumped EE's left knee on camera cover.
Employee was pulling pins to release beams, miner operator pulled beams back, beam caught pin and jerked pin forward causing employee to strain rt. shoulder
Employee was aligning holes on beam pedestal when it slipped from their hand cutting finger.
Employee was putting air in a dolly tire when the tire blew up striking employee's right arm
At 2:50 P.M. on 03/28/2024 Addcar system 18 became stuck underground approximately 480ft and is unable to move under its own power
EE was checking the fluids in the miner head, while turning the wrench EE felt something give in EE's back.
EE was working alone on the upper deck of the HW miner. EE said stepped on a roller. It rolled EE's foot out from under EE, falling and hurting their back. EE refused medical treatment. EE continued working until Sep.1st. EE sent us a letter from chiropractor dated Sept 8th stating EE was off under the chiropractor's care until Sep 11th at which time EE was supposed to see a Dr.
Parts were being hauled with a low boy trailer. Employee was moving in to start unbinding the parts and noticed a chain hanging down from trailer. As the trailer was being lowered to the ground, employee moved in to remove chain and the trailer lowered onto EE's right toes.
Changing coupling with 3/4" drive, while loosening bolts EE strained a muscle in EE's back.
Highwall miner was turning, something in the back right track popped and a piece of shrap flew approx 40ft striking EE in the right jaw below the mouth. there was an approx 3cm laceration with a small piece of shrap metal embedded that had to be removed at Pikeville PMC.
While doing repair work on miner push beams, Miner was welding the nuts holding pedestal on the push beam. When work was complete the miner slipped while stepping off, falling aprox 36" to the pit floor. while falling they reached out to catch themselves and landed on their wrist, breaking it in the process. Miner was wearing PPE which helped during the fall.
The employee was using a hand held drill with an auger bit to remove debris from a connector pin hole on a highwall mining beam. The drill bit caught causing the drill to spin around and hit EE's right hand.
Removing a roller from an Addcar conveyor car. The employee was using a six foot pry bar to remove the roller from the conveyor car. As EE applied pressure to the bar it slipped resulting in a strain to EE's right elbow.
The employee removed the pin that connects the conveyor cars together. As the loader began to remove the conveyor car from the launch pad the car shifted causing the pin to fall back in place. As the pin fell back into place the employee attempted to catch the pin. In doing so the end of EE's middle finger on EE's right had was pinched between the pin cap and the car.
While pulling conveyor cars out of the hole the employee was unhooking the electrical receptacle that powers the cars. As EE was working to unhook the receptacle EE placed left foot on the bottom rail of the car in the pusher assemblies path. As a result EE's left foot was caught by the moving pusher and pulled EE down on the launch base frame.
The employee was crossing the rear skid area of the highwall mining launch vehicle. As employee was crossing in the designated area employee's right foot slipped and resulted in a sprained ankle on right foot.
Replacing the bottom edge plate on a push beam. After cutting the plate loose it slipped and pinned EE's left hand little finger between the plate and the frame of the push beam causing the laceration to the finger.
The employee was placing a receptacle in the conveyor car tray. When placing it in the tray the employees left hand was caught between the receptacle and tray causing the left hand middle finger to be mashed.
Employee was setting up a car breaker when hand slipped off of breaker handle causing a sprain/strain to left hand. Employee continued to work until 11/12/2019 when employee went to doctor to have hand checked. Employee was removed from work by doctor on 11/12/2019.
Employee had lifted a lid and was looking at the trailing cables when the lid fell striking employee's right hand causing a contusion and swelling to right hand
MINER'S GLOVE BECAME ENTANGLED IN DOOR LATCH AS MINER EXITED CAB, AND AS A RESULT, THE GLOVE WAS PULLED OFF MINER'S HAND, CATCHING THE TIP OF FINGER, CAUSING A FRACTURE OF THE TIP OF THE FINGER BONE.
