The operator was accessing the cab of a CAT 633D Scraper. With the door open, the operator began to enter the cab when a gust of wind caused the cab door to close. The door impacted the operators back. More than 2 hours later, the operator reported EE felt some pain in EE's groin region.

Employee was utilizing a 12# sledgehammer to tighten a hammer union seal. Employee struck seal 2-3 times and then experienced pain in EE's right forearm and bicep.

Employee was on platform of 980 wheel loader cleaning the front window when EE slipped and lost EE's grip on the hand-hold. EE fell over the side, hitting the wheel well and then to ground. The wiper dispenser wasn't working well. The platform footing may have had dew causing the slip but not identified at time of inspection. Employee later complained that EE's back was sore.

A slow leak of a reagent tote spilled to ground while the plant was closed. The vapors/fumes were blown in the direction of the operator by the wind which was inhaled by the operator and upset EE's stomach. Later, the operator felt additional symptoms (headache, nausea) and vomited. Diagnosis was chemical inhalation irritation.

Employee was setting up a 10" by 8' plastic pipe (35-40 lbs.) under a loadout bin. As they started to lift the pipe, the felt a pain in right shoulder and set the pipe back down. Employee stated they didn't lift the pipe from the end and walk to a near vertical position as was common, but attempted to lift the pipe from the bottom 1/3 of length on opposite end. Reported 8/15/22.

The employee was attempting to access the cab of the Case skid steer. On EE's approach, EE tripped on the edge of the bucket and stumbled/fell into the bucket and struck EE's right thigh and forearm on the bucket.

Injured worker was holding an 8" bolt in left hand while another worker was striking head of bolt with a sledge. During a strike, the sledge missed bolt head and struck the injured workers left hand causing a fracture to the tip of employee's left ring finger.

The worker was walking down a slight incline on a hillside while inspecting a fenceline at an area away from all mining activity. While in stride, employee's lead foot (right) slipped on dry grass. As momentum carried employee forward, back foot caught on something and stayed in place causing a sprained left ankle.

Driver felt foggy and disoriented when truck finished being loaded. As EE drove away, felt hot and uncomfortable and stopped to get out for fresh air. As EE exited the truck feeling disoriented, stumbled from the step and fell to ground. A doctor said that dehydration and drop in blood pressure caused the disorientation resulting in the fall to ground and injury.

Plant Manager was in office when EE experienced chest pain. Weigh master called 911. Plant Manager was taken by ambulance to local hospital. Plant Manager never lost consciousness or needed CPR. Plant Manager underwent angioplasty procedure and received (2) stents to open artery blockages. Plant Manager was admitted to hospital and had overnight stay.

The worker saw what he believed to be a piece of slag on a steel support. While using a 4 lb sledge hammer to position an I-beam between 2 supports, he made contact with the side of one of the supports while hammering in the downward position. The area where he noticed slag was in fact a sharp edge in which he made contact with. Worker was wearing gloves at time of incident.

The operator opened a screen door in the screen house and as he was brushing the screen with a long-handled brush heard a loud noise and the screen door then dropped onto his right arm causing a contusion. The cable that is attached to the screen door to assist in opening the door broke from the counterweight causing the door to drop.
