After replacing V-belts on an elevated stone conveyor drive motor, the employee removed the lockout tag. He then returned to the motor to visually observe the belt tension while the belt was running. He went to retrieve a glove that had been left on the motor mount when his right hand came in contact with the belt and drive pulley, causing lacerations to his middle finger.

Powder in bulk truck jammed up in bin-EE climbed on top of truck to poke powder (break loose) pole was too long-he decided to lower hand rail to break pole-pole broke causing EE to lose balance-falling to ground. Broke left leg, left shoulder dislocated, bruised left lung.
Employee was assigned to clean up lime dust/powder off a concrete pad. He pulled a muscle in his lower back while using a shovel.
During the blast on 5/29/13 at 2:46pm - Fly rock escaped the perimeter and struck a representative of Pomps Tire Company in the right arm. The wound required 6 stitches to treat.
