Employee was helping move a 3" suction line. when EE entered the containment area, EE slipped and feet went out from under EE. The employee fell on butt first and rolled over onto right shoulder. EE stated was fine but the next day EE felt sore. On 7-1-16 EE decided that should go in and have shoulder looked at.

A contracted employee was climbing out of a semi-truck when he missed the last step falling to the ground. The contracted employee had his hands full and did not maintain three points of contact while de-boarding.

A miner was threading a 2" in diameter pipe. The rotating pipe became entangled with the miners sweatshirt sleeve, throwing the miner to the ground. The miner landing on his left shoulder. Possible left shoulder separation/sprain (waiting on MRI).

Injured was taking out the garbage. When he pushed down on the garbage can liner to compact the contents a tin soup can lid penetrated the bag cutting his hand.

Employee was holding on to the drive end of a pump while another employee was turning the pump impeller into the shaft. The employee's fingers got pinched when the impeller seated down on the shaft.

Employee was driving a Loader to the hopper when the right corner of the loader bucket caught the lip of the light post. Employee bumped his head on the load rite scale and cut his right eyebrow area.

Walking across Plant floor and rolled right ankle. Witness claims there wasn't a slip, trip, or fall. The incident was reported on 7/27/14 to the safety department. Diagnosed with a sprained ankle/foot. Employee received a x ray on 8/28/14 due to residual pain, and was told that a fracture may have occurred. Waiting for recommended CT scan and final doctor notes.

Injured was removing a bad magnet by hand from rev 1 lower deck with help from a co worker. They lost control of the magnet. Injured attempted to try and regain control causing what was believed to be a strained shoulder. Injured didn't feel medical attention was necessary at the time. Note: 4-8-15 notification was given that the injured would undergo surgery for a torn labrum.

Operate haulage truck, claimed driving equipment caused inflammation to scrotum. Employee was off 6 days - scheduled work days were March 3-6, March 11-14. Employee returned to full duty on March 18th. Question of work related injury. Validity of claim is being investigated

Individual was performing a test in the Lab on the Slon bench tester when the magnet was turned on and the fill tank came down on his right pointer finger. The finger was smashed and it caused an open wound on the inside of the digit. He received 7 stitches at the local hospital.

Two Hammerlund Construction 773 haul trucks were hauling fill for a road build on an internal impoundment. The bulldozer operator noticed the bank giving way and called on radio trucks to get out, one truck slid approximately 20 feet on its wheels and rested while the full truck flipped on its top and slid 20 feet. There were no injuries due to the incident.
