Slippery Step/ Slipped and fell on buttocks
Slipped off or missed step on truck. Rt Arm full of personal items, stepped down and missed a step. Grabbed the grab handle of the truck and pulled right arm.
Employee bumped left knee on conveyor belt; piece of the conveyer frame. Employee reported 3/5/21 that employee did it and was a little sore but would let supervisor know if it needed attention. 3/20/21 reported it was getting worse; not better. 3/21/2021 went to immediate care.
Employee was grinding. EE did not realize it was a piece of metal that went into EE's eye until the next day. EE thought it was dirt. EE was wearing safety glasses and did not think that the metal would have hit EE's eye. (Left Eye)
Cleaning bolt with Bench Grinder. Did not set Bench Grinder to correct clearance measurement before using Bench Grinder. Right Middle finger got sucked in. Fractured tip of right middle finger
Employee was weeding around the crusher and brushed up against poison ivy.
Employee was hammering in a pin and part of the pin broke off; pierced through boot and went into right shin bone.
Working underneath Conveyor belt; rock flew off and hit toe
Wires on ground. Employee walked from one area to another and tripped over the wires. Fell on ground straining right shoulder.
There was a rock stuck in the drop box tailgate. Went to remove the rock form the tailgate with hand, tailgate closed and pinched finger.
Climbed up on a conveyor belt to change product; slipped on small rocks; with a chain hoist in one hand, the hoist whipped around and hit worker on the mouth when worker slipped. Accident caused a broken tooth and caused some damage to bridge in mouth due to the broken tooth.
Employee was cutting into a conveyor belt with a saw knife and cut on left forearm. EE received several stitches at urgent care.
EE was shoveling out underneath a conveyor belt when he strained his back.
EE was doing clean up around the crusher. He went to pick up a rock, when it slipped out of his hand and landed on his other hand.
