EE was coming down steps from our plant when EE over stepped last step causing EE to sprain ankle.
Employee and c-worker were replacing drive shaft under floor of cab. Only way to access is from inside cab. Employee raised up too fast hitting his head on fire extinguisher mount.
Walking to equipment on said his back started hurting
On Oct. 31 it had been raining. Operator had mud on his boots as he climbed steps to loader, lost his footing causing him to fall backwards onto his right hand causing fracture.
Wrench slipped off of grease fitting in tight space causing him to hit left thumb and cutting it
Hit bearing with hammer causing piece of metal to hit eye.
