working on a 3 step ladder was attempting to tighten a bolt. Wrench slipped off the bolt forcing the IP to fall off the 3 step ladder landing on right side and elbow causing a cut to the elbow.
Employee was helping other employees place portable conveyor onto rail track to unload railcar. Employee's finger was caught between portable conveyor and rail track causing injury.
Miner was cleaning under conveyor with a machine (Vermeer)- when lifting bucket, bucket hit another conveyor causing the machine (Vermeer) to kick forward and up pinching miners hand between Vermeer controls and bottom of conveyor.
Miner was walking through the plant - Reached to open door into the floc room - left leg slipped from underneath EE causing EE to land on the right patella (knee).
Employee was removing the hose from the Hurricane 755 Industrial Vacuum, when EE was pulling on it the hose came lose the hose hit EE in the face causing EE to have to have 4 stitches above EE's left eye.
Employee was driving the JLG when they hit a pot hole causing them to bounce off the "safety foot peddle". This caused them to come to a sudden stop straining their back. Employee was brought in for evaluation and returned to work with restrictions, no lost time.
We were contracted to change tires on a 844 John Deere loader. In the process of jacking the equipment up - the blocking and jack failed causing the loader to shift downward and the employee was struck by the loader frame.
Employee was loosening a jam nut on a Rotex arm when the jam nut unexpectedly broke free causing the employees hand to slam forward into the rotex arm.
Employee when walking slipped on ice, causing employee to land on employee's shoulder.
Miner was coming down a flight of stairs - quickly and skipping steps. Miner caught foot on the bottom last couple steps and fell hitting miner's head.
Miner was closing the doors on a Rotex machine. While pulling down on the latches for the door the miner felt pressure and pain in the shoulder and forearm.
Miner was working on pulling a bearing off of a shaft. EE was inching it out to allow enough space to place a strap on, but the grease on the shaft caused the cover plate and bearing to slip off and fall. The bearing and cover plate landed on the top right of miners foot.
A large piece of frozen sand caught on half on the belt and half off causing plugging. Employee removed the chunk of ice off the belt and placed on the ground. Employee did not feel any pain, but yet felt something that wasn't right in the groin area.
Miner was walking from one building to the adjacent building when miner lost EE's footing and slipped and fell. Miner was taken in to the emergency room due to pain in EE's left elbow area.
Employee was shoveling sand onto a conveyor belt. Employee was cleaning underneath the conveyor. As ee was lifting the sand to put on the conveyor ee felt left shoulder pop. This causing pain in shoulder.
Miner was replacing a valve that was worn out using an adjustable wrench. Miner had 4 bolts to remove - upon EE's last bolt to remove the valve dropped causing a laceration on EE's left forearm.
While changing bucket teeth a metal fragment struck Miner's arm.
While wheeling a 5" roller into place for as a replacement part for a belt press the roller came off the cart causing the individuals finger to be caught between the roller and floor grating. The individual suffered a break in the middle finger.
While lifting a 20# LP Tank the individual felt a pain in the lower back region. This individual was then suffering from a lower back strain.
While operating a haul truck the individuals tires slipped into a rut causing his head and neck to have a whiplash type effect.
While setting up water pumps in the mine the individual stepped on unleveled ground casing the individual to twist his knee.
The individual was clearing material from the scalping tower location. While doing this clearing the individual attempted to pick up a large rock to remove it from the area, while lifting this rock it caused the individual back pain.
While reaching for the door handle on the quality control lab the individual received a bee sting on his left hand.
The employee was in an Aerial Lift working to clear a troughing roller utilizing a hammer. The equipment was not locked out at the time of incident causing the hammer to be drawn into the troughing roller also drawing the employee's hand in between the belt and roller. The employee received a hand contusion as a result of the incident.
The individual was carrying a 5 gallon fuel can to refill a heater located in the second level of the scalping tower. This individual reports slipping on ice while accessing the tower to refuel the heaters.
The miner was injured while shoveling sand into a 5 gallon bucket and lifting the bucket from feet level to eye level. Contributing factors include twisting and bending while lifting the bucket.
While moving a wheelbarrow to empty it the individual twisted/strained his lower back. Stated that the concrete was covered with sand. This incident was not reported to safety until 12/29/2014.
While working in a Scissor Lift cutting a threaded rod with a reciprocating saw, the saw blade caught causing the saw to kick back impacting the operator's face/upper lip region causing a cut/laceration.
Driver was adjusting tailgate chain and box was slightly raised, chain moved pinching his two fingers.
