While washing the cab window on a DM45 Rock Drill an employee stepped into an open deck sampling hole with his left leg. The employees right leg slid out from underneath him causing injury to the employees right leg. The employee was taken to the ER and assessed for injury. Employee was released back to modified work with a request for the employee to seek care from a orthopedist.

Showing all 3Mechanic cut fingertip using razor blade. While first aid was being given he passed out due to the sight of his own blood and struck his head as he collapsed. Was revived and transported to local clinic. They couldn't treat him and he was flown to UMC Trauma in Las Vegas. Cat scan was performed and read in Las Vegas. He was then released to return to work at 2:15 pm that day.
Mechanic was repairing seal on hydraulic ram block. He disconnected hose and pushed it out of the way. The hose then slipped and struck the mechanic in the tooth.
