Showing all 4Loader in pit, mucking out shot. Operator dismounted seat. When operator remounted they fastened seat belt and began to dig. Unfastened seat belt to readjust. Large rock slipped out of bucket while they adjusted the belt. Operator was tossed forward slightly, striking forehead into rops bar. Inflicted gash on forehead. Five stitches needed. Miner returned to work next day.
IE was scraping dust from underneath a conveyor. IE did not shut door & lock out the conveyor as required. Conveyor belt pulled sleeve & IE's left hand into the roller & mashed it.
Employee was walking on the ground around base of crusher at the plant. Employee was looking up at quarry foreman & plant operator. Employee stepped backwards on a rock & lost balance. Employee fell down & right lower back landed on another rock, approx. size of softball.
