Middlesboro Mining Operations Inc · Struck by falling object

Employee was on 2nd floor waiting for a 250 gallon chemical tote to be raised and the metal frame on the tote broke and fell on employees hand which was on the handrail. Laceration to right hand requiring surgery to stitch.

Middlesboro Mining Operations, Inc · Fall from machine

Driver was descending a hill at mine, loaded with coal, when suddenly EE lost air pressure, causing loss of the braking system. EE jumped from the truck, landing on the roadway causing lacerations and possible cracked shoulder.

Employee was on bank reclaiming with excavator and the excavator became unbalanced tipping several times over the spoil, landing on it's side. Employee transferred to hospital by ambulance, then flown by helicopter to U.T. hospital.

Employee was trying to move chain on drill into place using a pry bar. The pry bar slipped striking employee on face causing a laceration. Employee was taken to E.R. where they glued it closed. Employee returned to work.

Operator was pushing uphill on slope and noticed pain in upper part of back area. Went to Dr. for x-rays. Shows signs of spurs on spine, which was inflamed.
