The miner was walking through shop prior to shift, to put lunch in the refrigerator. The shop lights had not been turned on, yet. As EE rounded the work bench, EE tripped over an air brake pod that had been stored under the table for future use. The miner was not able to catch self with hands and fell onto the right arm/side and hit face on the floor.

Miner was on top of TAMKO bin shoveling 1 1/4"X 1/2" aggregate that had overflowed onto the bin top. When going into the rock pile with shovel, the miner felt a sharp pain in elbow.
Miner unloading over-loaded trailer, was entering trailer second time when miner realized trailer was moving away from dock, Forklift was 1/2 in 1/2 out, and slid back and fell against dock plate then at angle to ground, when trailer stopped leaving forklift at an angle, with rear counter-weight end on ground, forks in trailer. Miner dismounted equipment.
Miner was operating a CAT D-350 Articulated Haul Truck, hauling C-4 Dust to Stockpile. Upon returning in Snowstorm to Line 2 Load-out, Right Front Drive wheel slipped off roadway into a drain depression, jarring the driver in cab and causing later back pain.
Miner (Maintenance - M5) was with Manager of Fine Grind/Mill Operations and was attempting to cut thru a piece of conveyor belting. Miner had noticed his work utility knife had a dull blade, and upon changing it with new one, was cutting belt when knife skipped/slipped and cut his right lower leg. Miner was transported to OCCU-MED and 4 sutures were used to close small laceration.
Miner is a Mill operator and was operating a C4S Packing machine. Miner was palletizing 50 lb. bags of C4S material when he felt a sharp pain suddenly in his left forearm area. Miner was taken for medical evaluation, and placed on lifting restrictions until a scheduled MRI X-ray can be conducted. Miner is working w/restrictions.
