Miner was using a handheld grinder on a weld when metal debris flew into left eye. Eye wash station was unsuccessful at remedying, so miner was taken to urgent care for treatment. Doctor successfully removed metal debris with tweezers.
Electrician had the dredge control panel propped open. The prop failed causing the panel to slam down striking EE's left arm just above the wrist. Urgent Care assessed injury as a contusion with no broken bones and directed right handed duty only until follow up appointment to reassess the injury.
While installing a timing belt on a screen, the counter weight came down pinching employee's middle two fingers on left hand between counter weight and flange. Diagnosis: Open fracture; injury of nail bed of finger.
The employee was clearing a material jam from the jaw crusher. EE lost EE's footing and fell onto the deck with EE's ankle bent beneath EE.
Employees were moving a divider box with a front-end loader and chains. One of the chains broke and stuck the employee in the arm.
Employee slipped on ice, fell and struck EE's head on the ground. Initial complaints were of pain in EE's head and right shoulder. Several days later, EE still didn't feel well and went to the ER. Possible head injury, will be off work at least 5 days.
On 9/27/24, employee was tightening 5/8" bolts with two wrenches when EE felt a sharp pain in the wrist. EE's wrist and hand became swollen. An x-ray revealed EE had a small piece of metal embedded in their hand from a scratch EE received the day before. Medical professionals removed the metal from EE's hand. EE's wrist was only strained.
Employee was using a hammer to remove an old bearing from a crusher shaft. A sliver of steel broke off and entered the employee's leg. An x-ray showed the metal inside employee's leg, doctors opted not to remove it at this time.
Employee was dismantling a piece of equipment on the floor of the shop, EE stuck the back of EE's hand on a nearby wooden pallet and received an 1 1/2" long sliver in the back of EE's hand. Medical personnel cut the skin to push the sliver through to remove it.
The employee spent the day doing various maintenance jobs welding and grinding. EE experienced eye irritation and thought it was due to smoke or dust. Late in the evening, EE went to the ER and had a small piece of steel removed from EE's eye. EE reported the injury to EE's supervisor on 4/8, the supervisor forwarded it to the safety department on 4/18.
The employee was asked to help pull down on a pry bar positioned overhead. The pry bar slipped and struck the employee in the head. Employee was not wearing hard hat.
Employee was adjusting a feed chute to prevent spillage. The chute is suspended by two chains. When the employee loosened one chain, the chute swung over and struck employee in the head. Employee had left hard hat in the truck and was not wearing it.
Employee was removing the rear panel from a screen, lowering it with a chain come-along. The panel rotated and pinned the employee's leg against a nearby pipe.
Changing screen and finger got pinched between bolt lock and side plate.
Employee was assisting with maintenance activities. EE was lifting a pulley shaft out of the bed of a truck and dropped it on their foot.
Truck driver forgot to latch the tailgate before being loaded. EE tried to clear stones from the latch by hand. EE's fingers got pinched by the tailgate and lacerated by the latch.
Employee was traveling in the basket of a manlift when something caused the lift to unexpectedly increase in speed, throwing the employee off-balance. EE struck EE's ribs on the handrail of the man basket.
While shoveling material off of a radial stacker, the employee reported feeling pain in abdomen that continued throughout the day. After following up with the physician, EE was diagnosed with a hernia on 8/27/21. Repair surgery is scheduled for 10/14/21. EE currently has not lost any time and is not on restricted duty.
employee used a cigarette lighter instead of a striker to light a cutting torch and burned EE's hand.
Employee was cutting material for belt splicing, the knife slipped and sliced EE's thumb. Stitches were required to close the wound.
EE was driving a haul truck when EE struck a pothole in the haul road and reported back pain. EE left work at 11:00 am, called off the next two days and saw a physician on 07/14/2020.
Company electrician was lifting a 40lb roll of wire into a van from the ground and injured their shoulder. Electrician did not lose time at the time of the incident, but needed surgery to repair the injury on 11/28/2018.
EE was backing the yard truck under the finished product bins to load out. While turning the wheel, EE caught finger in the vent window latch and dislocated it.
Employee was bending over to put a steel cover on a pontoon when EE felt a sharp pain in EE's lower back.
Employee was clearing vegetation around a building on the mine site when they suffered an allergic reaction resulting in a rash on the arms and torso.
Employee was using a utility knife to cut rubber belting. The knife slipped and cut the employee's leg. Stitches were required to close the wound.
Employee was shoveling snow when EE slipped on ice, fell and hit EE's head on the ground.
Employee was using a 115-ton Ironworker to cut miters on steel angles. During the job, the dies shifted until they came in contact with each other, causing a shard to break off the top die, which struck the employee in the left side of neck, requiring surgery to remove it.
Employee was using a utility knife to cut conveyor belting. The knife slipped and cut the employee's finger. Surgical glue was required to close the wound.
Employee was descending from an excavator after fueling it. Employee's foot slipped off the steps. One hand was on the railing, which was left supporting all of the weight of employee's body, causing an injury to employee's shoulder.
Employee was cleaning out trailer and somehow fell and fractured wrist. Accident was reported to management on 4/10/17.
Injured employee was changing screen panels and cut a finger on the Unistrut channel that holds the panels in place. Employee needed to go to an urgent care facility where Dermabond was used to close the wound.
Two employees were hand-loading a fuel pump into the bed of a pick up truck. As they set the unit down, One employee had finger crushed by the pump base. EE had surgery to repair the finger on 11/17 and returned to work on 11/21. EE is currently on restricted duty.
Delivery truck driver was climbing over the side of the truck box to enter the bed when ee missed a rung on the built-in ladder inside the box and fell, suffering a fractured rib.
Employee was welding while standing on a ladder. The ladder was not secured properly and slipped out of position causing the employee to fall, injuring knee. Employee has been off work since 4/15/16 and has an appointment for an MRI later this week. Will be off until further notice.
A storm was coming in and the wind had picked up. Employees were cleaning up the work site in preparation when one employee had a bit of sandblasting sand blow up under his safety glasses and into his eye. Flushing with water did not remove it so he saw a physician who removed it and prescribed eye drops.
Employee was stripping a belt to be vulcanized. He was using a knife to cut the cords and cut his thumb.
Employee was pulling weeds and trimming brush over the course of the day. Later began itching and developed a rash on her forearms. The plant that caused the reaction is unknown.
Employee was turning a 4-sided die on the press brake. One end of the die slipped off the support chain and crushed the employee's finger.
3 employees were assembling conveyor sections to extend the field run. One section was not lined up properly. An employee attempted to pull it into alignment by hand & felt pain in his abdomen. He didn't immediately seek treatment. He later saw a doctor & was diagnosed with a hernia & will be scheduling surgery. He reported the injury on 5/23/14.
