Showing all 5Miner was standing on a conveyor catwalk and leaned a piece of 1/4" thick, 3" angle approximately 46" long against the handrail. The piece of angle fell on its side on the walkway with the corner of the angle landing on the miner's foot just behind the steel toe section of the boot.
MINER WAS WORKING ON A PIPE, PARTIAL PART OF A DIRT WALL FELL AND HIT THE MINER'S LEG.
Worker attempted to loosen a bolt with a wrench, hand slipped off causing employee to fall from their feet.
While stockpiling a 769D haul rig, sunk on the edge of a stockpile and slid down the face of the stockpile a few feet. The Miner was retrieved using a man lift, and the haul rig was lowered the rest of the way down the stockpile using two 992 loaders. The miner did not have any injuries and was checked out by EMT's as a precautionary measure. The haul rig sustained no damage.
