Springfield Coal Company, LLC · Fall onto or against objects

Employee climbed into bed of semi with a shovel to break loose carbon that was stuck to the bottom of the bed, while using the shovel employee slipped and fell on right shoulder. Employee was taken to Express Care for evaluation, after x-rays it was determined no bones were broken. They recommend seeing an orthopedic doctor.

Springfield Coal Company, LLC · Over-exertion in lifting objects

Mechanic was assisting the welder in picking up a plate of metal (4'x10'x1/2"). Two plates were stacked on top of each other on the ground. Employee tried to lift top plate so the forks of the bobcat could slide under top plate. When ee attempted to lift this plate, ee heard a popping sound from the lower right side of back. Employee collapsed onto the ground in pain.

Pittsburgh Energy LLC · Fall from machine

Employee was being task trained on the operation of the water truck. Employee stated that while dismounting facing toward the inside of the cab of the vehicle, pant leg got caught on the edge of a bracket. When the pant leg caught, employee lost grip on the handle and fell to the ground. Employee landed on right arm, resulting in a bad sprain and muscle contusion.
