Maintenance worker was changing HSI hammers. When EE was rotating the roster to change the third hammer, EE's foot was pinched between a new hammer and the main frame, fracturing EE's foot.

Employee was trying to remove entangled conveyor from pulley on QH331 Cone. Employee was using a pipe wrench to turn the pulley belt, when ee hand slipped catching ee finger in between the pulley and the guard. This resulted in a cut right index finger.

Employee was grinding slag off a piece of 1/2 in metal plate, when a particle passed through the gap of employee's safety glasses and face getting stuck in eye.

Track roller had come off of Drill Rig. While attempting to reinstall the roller by hitting it with a hammer, a small fragment of metal came off cutting through the employee's glove and cutting EE's left finger and imbedding under skin.
