Employee was getting off the bus at the Underground to report for pre-shift lineout. Upon exiting the bus, the employee twisted their right ankle.
While an employee was attempting to charge a hydraulic valve, the fitting/connection failed on the lower hookup and there was a pressurized release of force. The employee sustained injuries to their left ear, where a small piece of metal became lodged and was removed with minimal force. The employee also injured their left hand, but no fracture was identified in the x-ray.
Surveyor was marking lines on shot muck when a rock rolled onto their arm. Then rock forced their hand into another rock causing injury.
Employee stepped off of water truck and rolled right ankle causing injury.
While exiting the cab of an articulated haul truck, employee stated EE's knee made contact with the vehicle's driver's side door panel. Employee was treated onsite by EMTs and released back to duty.
Employee was using a crane outfitted with a magnet lifting device attempting to lift a cutting edge from a dozer. When the magnet was attached, the employee stuck EE's face near the magnet and the handle released and hit EE in the nose causing injury.
A fueler was reversing the fuel truck and contacted a rock causing pain in the neck and shoulders.

Process supervisor was pulling a water hose when it became caught on something and caused the person to lose balance and fall. A restricted injury occurred.
Warehouse leadman was assisting move ground engagement tools onto a pallet using a lifting magnet. When EE released the magnet and stood up, EE lost balance/slipped and fell, resulting in an injury to shoulder and knee.
Equipment operator was exiting the rear passenger seat of a pickup truck and stuck their knee against the door causing injury. Operator was place on restrictions.
The transport vehicle employee was riding in hit a bump. The employee noticed a pop and pain in right knee as their legs were tossed from the bump.
Process plant metallurgical technician was carrying samples in 5 gallon buckets when their back was strained.
The operator was driving a surface loader and hit a bump, which jostled them. They didn't immediately feel pain and finished the shift, but at the start of shift the following day they reported soreness in their back to their supervisor. They were sent to the hospital, where the doctor wrote them out of work for two days to rest their back muscles.
Mechanic was removing teeth off of a bucket using an 8lb sledge. There was no specific incident of injury but the mechanic explained that EE's elbow became sore, EE's grip strength became diminished on the right side, and pain worsened through the night.
Employee was walking a semi-cleared footprint for a future embankment and tripped in a small hole. They twisted an ankle and was taken to the site EMT and received first aid for a sprain. After traveling home, employee visited a clinic and a break in their fibula was identified.
A blast driller experienced an STS in EE's right ear confirmed by a retest that was reviewed by an audiologist. An internal investigation revealed a single event more than likely contributed to the STS. The event took place a few weeks prior to the initial hearing test. The blast driller was outside of the drill when air pressure was released while not wearing hearing protection.
A Mine Operations employee was riding the bus out to the 601 Park Up area when the bus hit a bump in the road causing a back injury to the employee. The employee was a passenger on the bus at the time of incident
Drill mechanic's right pinky and ring finger were injured when a shock sub fell from the rigging contacting EE's hand. The mechanic was in the process of changing out the shock sub for the drill.
The operator was setting up to load/charge a round when the emulsion hose became stuck underneath the primer box lid. When the operator leaned over and moved the hose out from under the lid, the lid dropped down and pinched EE's testicles between the lid and the primer box.
Blast drill technician was assisting in loading a new hammer bit onto the deck of the drill when the hammer slipped pinching EE's finger causing injury.
An UG haul truck operator injured EE's back while dumping CRF material into an underground CRF bay. While the dump bed was raised up, it scraped the "back"/roof of the CRF bay, causing the cab of the truck to lift off the ground and then fall back to the ground.
Employee was on the shot pattern for shift that began at 8:00 am (excluding rest breaks and water breaks) and experienced cramping in legs and hands due to heat exposure at 12:30 pm.
Process operator received a right pinky injury when a pipe wrench fell from a worktable. This incident caused a restricted injury.
Carrying out maintenance on a Sandvik 431 Cable bolter. removing built up grout from bolts was a hammer and bar. Hammer handle has broken mid swing personnel has hit hand on the bar and machine causing an acute fracture to middle finger on right hand.
Process plant employee was performing clean-up work when they became overheated.
While installing a heavy iron liner into the mixer at the batch plant, the employee suffered a muscular injury in their left bicep.
A maintenance employee was welding on a haul truck bed in wet conditions and received a shock. As a result, a physician has placed the employee on restricted duty.
A maintenance technician was performing work on an UG haul truck in the maintenance shop (located on surface). The mechanic needed to remove a rock that had fallen onto the top deck of the truck. While removing the rock, the rock slid out of their hand, causing a laceration to right thumb. The operator was wearing nitrile gloves.
Operator was assisting with metal cutting and handling. Afterward, EE felt irritation in EE's eye. EE attempted to flush EE's eye, but EE was unable to dislodge the object and was taken to the emergency room for treatment. The object work itself out on its own, but prescription antibiotics were prescribed (precautionary)
Operator was leaning out of the cab of a mucker and was bracing self with EE's left hand against the rib when several rocks broke free and struck EE's left hand and arm, causing a laceration to EE's middle finger that required 10 stitches.
A maintenance mechanic was troubleshooting a hydraulic leak on a Cat D10 dozer under the floorboard area. During this inspection, pain was felt in the knee that resulted in restricted workdays.
Employee was loading a rock crusher with excavator and noticed crusher was jammed. The foreman turned crusher off, and operator waited for the machine to stop. Then, tried to adjust the jammed rocks with their hands. The employee used smaller rocks to push on the larger rock to try to free it. The larger rock(s) shifted and employee's hand was caught between crusher frame and rock.
An UG maintenance mechanic was using a soft steel hammer to break free a bucket tooth (G.E.T.) from a 9 yard loader bucket. When the soft steel hammer made contact with the tooth, a small metal shard sheered off and imbedded in the employees right leg calf muscle.
While washing the cable bolter, the operator had some grout come off the equipment and splash into their left eye. The operator flushed eye with a bottle of water and continued working. An hour later they told supervisor, and stated that they had no residual issues or pain. The following afternoon, they woke up for their shift and the eye was red and weeping.
While loading bolts from a pallet to a truck, the operator twisted an ankle but continued working and proceeded to move the truck to a new area. While at this new location, employee was removing the banding from a bundle of bolts, and the front stack of bolts fell onto the same ankle/foot, causing a sprain.
While trouble shooting a hydraulic leak on a blast drill, a maintenance technician was injected with hydraulic oil in their left index finger.
A process maintenance worker was lifting the corner of a 1/4 inch plate of steel to allow the forklift to pick up the plate when worker felt pain in EE's back.
A maintenance welder was cutting plates from a haul truck bed in the rebuild process. While cutting away a section, there was stored energy that released and the plate struck the welder in the left arm and face. The upper part of the left arm received soft tissue damage including bruising and swelling.
The miner was charging a hole in the back and was using a spider plug/pilgrims hat to plug the hole. When EE inserted the plug and pulled the PVC pipe out, a rock was knocked loose and hit the miner in the face.
Employee was using a small crowbar to pry off bearings to prepare to line bore. Crowbar popped off the bearing and the bearing fell off striking the employee on the inside of the left knee.
A maintenance technician was welding on a haul truck bed in a squatting position. When they went to stand, they twisted and felt a pop in their big toe.
Injured worker was attempting to cut through a seam weld on existing geomembrane liner with a utility knife when the blade slipped loose from the liner and struck the top of the thumb on the opposite hand that was holding the knife. The hooked blade penetrated the worker's leather glove. Several sutures/stitches were required to close the wound.
Equipment operator felt a "pop" in knee when exiting the operator's haul truck. The operator has been placed on restrictions.
A process operator received lime burns to abdomen, back, and face when they came into contact with lime. The operator was removing a bolt on a flange at the lime slaker.
Operator was carrying split set bolts when EE tripped and fell hitting EE's elbow on the fender of the jumbo drill causing injury.
A process employee was placing a conveyor guard back into position when the guard slipped smashing finger. The injury resulted in a fractured left ring finger.
A surveyor was walking on a shot muck pile to put in stakes for dig control when the surveyor lost footing and fell. A leg injury was the result of the fall.
Nox exposure. Short of breath and coughing. Employee taken to hospital and spent the night then was released and returned to work with light duty.
