Showing all 3Employee was involved in setup of a pump at a sump location within the pit. Pump pad failed and pump was tipping into the sump. Employee tried to stop the pump from going in the sump. Employee failed to do so, pump carried backwards and sideways and struck the employee mid torso and causing injury to the their back
Employee was holding the head of the bolt with right hand while operating a impact wrench removing the nut on the bolt with left hand. The head of the bolt turned in right hand causing a laceration to the hand. Employee was taken to the ER where they received medical treatment. 6 staples were put in the hand to close the wound.
