The employee fell against I-beam over the pit, striking right shoulder. Impact from the fall caused dislocation of the right shoulder.
Customer truck driver sustained minor burns to the hands and face and swallowed a small amount of antifreeze when checking the coolant levels on their truck, which was running hot.
Operator was dumping load on soft, uneven ground. Load shifted while bed was raised. Truck slowly turned over onto side. Operator exited cab under own power.
