While walking to another area to weed eat around tank house they slipped on loose dirt & spread eagled legs on either side of the berm causing them to fall forward and hit their face shield & hard hat on the ground causing their neck to pop backwards as well as their groin to hit the rocks/gravel on the berm. It was hot and dry conditions that day.

EE & co-worker were fixing a fence, co-worker was holding the star picket post with foot & EE was hitting the post with a 20lb hammer. EE misjudged where the post was and skimmed right hand against the top of the post. The sharpness of the metal post & the momentum of EE's swing compounded the severity of the wound.

While cutting board with skilsaw the saw kicked back and made contact with the employee's left thumb causing a laceration. Employee was transported to hospital and laceration was treated with sutures and employee was released to normal duty.
