Employee was using a shovel to clear fines. The employee was found unresponsive, sitting with back against a wheel of the crusher. There were no physical signs of possible causes nor were there any witnesses. Employee is still being evaluated and tested for medical concerns.

Showing all 2Employee disregarded direct orders during maintenance, left group and walked around to a belt, removed the safety cover and was manually rotating the belt when power was restored to the belt. Employee's left thumb was sucked between the belt and pulley causing a severe laceration. Due to the severity, the thumb was further amputated at the hospital.
