Worker was working at the south or rear side of hopper in the main pit. A ball of clay came off the hopper and hit worker in the back of the neck causing EE to lose EE's balance and fall. Worker was immediately taken to the ER by the plant manager. Worker only sustained bruises from injury.

Showing all 5Employee was standing near main shaker screen observing operations. A piece of oversized rock was ejected out from main shaker and fell approximately 60 feet striking employee on crown of head causing laceration. Laceration was closed with glue.
Employee was helping install discharge pipe on sand pump. Trackhoe operator raised pipe so gasket could be installed between flanges. Employee was holding a bolt in place. The pipe shifted pushing the bolt between EE's ring and middle fingers on EE's left hand. Hand was lacerated and required two stitches.
Emp was cleaning area around and under conveyor/tailpulley with shovel. The conveyor was operating. EE allowed shovel to become engaged with conveyor belt/pulley. Emp got off balance and hit left eye on 2" diameter 2' long pipe on conveyor frame. Emp completed shift. Did not report for work on 5/13 due to swelling in left eye. Returned to work on 5/14. Eye is fine.
Employee attempted to do a spot weld without properly positioning welding helmet. EE suffered irritation to both eyes. EE was welding on a gravel trailer.
