The miners were trying to switch out a 10" x 50' poly pipe that had ruptured with a Caterpillar excavator. EE had put the chain choker on the pipe so it could be removed from the ditch and was trying to exit the ditch at the steepest end and in doing so lost footing and fell down twisting left knee.

Showing all 6Miner was attempting to tighten the packing on a GIW slurry pump when the Channel lock pliers miner was using came in contact with the rotating shaft causing the Channel lock pliers to snap up and pinching miner's right thumb and ripping/bursting the skin open. Miner received 10 stitches at the emergency room.
The miner was starting up the dredge and while doing so was on EE's phone and did not observe that the water nozzle was coming around to the right too far. The water struck the side door of the dredge and the miner knocking EE down and causing EE to strike the side EE's head on the microwave oven. This caused a laceration to EE's scalp of 6 inches.
The operator went out to the manual water cannon to reposition it and slipped. EE was trying to stop the fall and grabbed a beam to hold self up and in doing so twisted around fracturing the humerus near the shoulder. We have installed a no slip surface in this area and ordered a remote controlled nozzle.
Miner was changing the bag on the rotary airlock at the base of the dust collector. EE was trying to clear a bridge and removed the guard and put EE's hand in the airlock to clear the bridge without ensuring that EE locked out the disconnect. While doing so, the airlock actuated and EE's hand became caught between the blade and the side wall.
Employee was working in a de-energized 4160 volt GFCI controller in an attempt to reverse the polarity of the wires when it somehow became energized.
