A miner was struck in the ribs with a drill steel that was hidden under the muck in a horizontal position. When the backhoe operator raised EE's outrigger it grabbed the steel causing it to twist up out of the muck striking the miner (right side ribs).
Struck by rock that fell from top and clipped employee hard hat and arm causing laceration to EE's left forearm. Compliance with rules/regs, PPE, mining equip/systems not a factor. Miner proficiency may be a factor.
Employee was removing dollies from monorail when EE's finger got caught between 2 wheels. Mining systems was not a factor, compliance with rules and regulations was not a factor, PPE was a factor.
Employee was using a pick to cut a coal sample from the rib. While swinging the pick into the rib, a small piece of coal flew back and hit EE in the mouth, causing a small cut to EE's upper and lower lip. Employee received one stitch in EE's lip. Returned to work EE's next shift.
Employee was walking along the 7 North Track and the top of EE's hard hat struck the head of a roof bolt in the mine roof. Has pain in EE's neck. No rules or regulations were violated. Employee has training and experience.
Was traveling in shuttle car back to miner when a bent rib strap entered the kitchen striking EE on the back of the head. Compliance with rules/regs, PPE, mining equip/systems not a factor. Miner proficiency may be a factor.
Employee was changing a piece of belt structure when the frame slid, and EE's finger was caught between bed rail and frame. PPE was not a factor, mining systems was not a factor, compliance with rules and regulations was not a factor.
Injured was helping 2 other miners re-rail a 555 scoop at the 230 foot mark of the slope track. Two airbags were placed to lift the scoop, blocking was being installed to support the scoop to deflate the bags when a crib block shot out from underneath an airbag and struck the injured employees hard hat, resulting in a small contusion to EEs forehead and a sprain/strain to EEs neck.
Employee was installing roof bolts on the right side of the continuous miner. EE had completed drilling a hole and was pushing a roof bolt up in the hole when EE felt a "pop" in EE's right shoulder. No rules or regulations were violated and EE was wearing required PPE. Body failure performing normal job duty.
Employee was working on air lock doors when a rock fell from roof and struck EE's lip. PPE was not an issue, mining systems was not an issue, compliance with rules and regulations was not an issue.
Employee was putting supplies on miner when EE picked up a rib strap when it cut through EE's glove. PPE was not a factor, mining systems was not a factor, compliance with rules and regulations was not a factor.
Employee was helping unload structure from scoop when EE dropped structure on EE's foot. PPE was not a factor, mining systems was not a factor, compliance with rules and regulations was not a factor.

As they were rolling a rail, EE stopped but employees on the other end continued to roll their end, causing the bar EE was using to swing back and struck EE on the right side of EE's neck and jaw. Failed to maintain control of the hand tool EE was using.

An unplanned roof fall occurred on 5/4/26 at 8:22 am. The fall measured 16'by 40'long. The fall occurred in #6 entry 83 cross cut in 5 North. The area was posted on all sides and dangered off.
Employee was carrying 20' beams through mud. The employee got EE's foot caught on a cable bolt that was sticking out of the mud, causing EE to fall with the beam in EE's hands. Employee fell to the ground, crushing EE's left hand under the beam and EE's body weight.
Employee was walking down the face when EE tripped over a rock and injured EE's knee. PPE was not a factor, mining systems was not a factor, compliance with rules and regulations was not a factor.
Employee was scaling rock down from top when a piece of rib started to come down. While EE was moving away from rib EE slipped and fell injuring EE's knee and ankle. PPE was not a factor, mining systems was not a factor, compliance with rules and regulations was not a factor.

Employee was at the miner hanging cable and water line into hanger when EE went to pull down on cable, EE felt pain in EE's shoulder. PPE was not a factor, mining systems was not a factor, compliance with rules and regulations was not a factor.
Employee was exiting a mantrip when EE's right foot was stuck under the seat. When attempting to pull it free EE injured EE's right knee.
Employee was helping pull belt off of dolly when the belt grab slipped off belt and struck EE's nose. PPE was not a factor, mining systems was not a factor, compliance with rules and regulations was not a factor.
Employee was cleaning slate off of the top of EE's canopy when another piece of slate came down and hit slate bar and a piece hit EE's face. PPE was not a factor, mining systems was not a factor, compliance with rules and regulations was not a factor.
Employee was walking to center bolter when EE slipped and fell, EE tried to catch self and cut the top of EE's hand on a piece of slate. Mining systems was not a factor, compliance with rules and regulations was not a factor, PPE was a factor.
Employee was installing center bolts using the Fletcher Roof Bolter. Had drilled EE's starter hole and just changed drill steels and when EE started drilling again, a piece of rock fell from the top and bounced off the drill head and struck EE's left forearm. Suffered a contusion with swelling. Missed 3 days of work. Has training and experience and was wearing required PPE.
Employee was helping work on scoop battery when battery lid came down and hit EE's elbow/bicep. PPE was not a factor, mining systems was not a factor, compliance with rules and regulations was not a factor.
The rope gripper wear pads on Metz elevator wore down causing a pad wear microswitch to engage that causes the rope grippers to set, resulting in an entrapment of 2 miners approximately 350' down the shaft. The manual rope gripper release pump failed. The miners were removed from the elevator at 5:45 pm. No problems with equipment or compliance issues, training or PPE.
IE was changing a roller on the slope belt. The chain hoist supporting the roller slipped and IE's finger was caught between the roller and the frame. Injury was not the result of a training or compliance issue.
IE was a passenger in a mantrip that was travelling into a track spur. The mantrip struck a dolly that was unseen by the driver, and IE's back struck EE's seat hard. Employee continued to work without restriction until seeking medical attention on 4/24/2026, and began missing days after that. Injury was not the result of a training or compliance issue.
Injured employee was carrying a belt roller with both hands along the 7 North #1 Belt between 24-25 blk. when they tripped on a piece of loose coal and lost balance. They fell against the side of a roller cradle, smashing right hand between the cradle and roller they were carrying. Suffered a contusion & swelling. Was wearing required PPE including metacarpal gloves.
Employee was drilled hole for rib pin and turned around to grab strap when piece of rib hit EE's back and shoulder. PPE was not a factor, mining systems was not a factor, compliance with rules and regulations was not a factor.
Employee was loading waterline onto scoop for belt move. When one of the water lines pipes slid off the scoop and struck EE's foot. PPE was not a factor, mining systems was not a factor, compliance with rules and regulations was not a factor.
Employee got EE's left hand caught between the auger and wrench while rib pinning, resulting in a laceration to EE's left hand. Compliance with rules/regs, PPE, mining equip/systems not a factor. Miner proficiency may be a factor.
Injured was cutting down roof screen to unload "bagged" material at 3 block on 3 belt with a grinder and some loose rock that was in the screen fell out causing a laceration to the posterior side of EE's left forearm that required sutures to close. No problems with equipment or compliance issues, training or PPE.
Employee was cleaning on LW face when another employee was attempting to pass EE, that employee slipped and fell and the miner that was cleaning tried to catch EE and injured EE's back. PPE was not a factor, mining systems was not a factor, compliance with rules and regulations was not a factor.
Employee was struck by a rock that had fallen from in between the shields, striking EE's left hand, and resulting in a contusion to EE's left pinky and ring finger. Compliance with rules/regs, PPE, mining equip/systems not a factor. Miner proficiency may be a factor.
Employee was working on LW face at 12 shield when rock came over handrail and struck the back of their leg. PPE was not a factor, mining systems was not a factor, compliance with rules and regulations was not a factor.

Employee was working on a cable repair when knife slipped and cut their finger, causing them to receive stitches. PPE was a factor, mining systems was a factor, compliance with rules and regulations was not a factor.
Employee was at the miner when a piece of rock fell from between straps and struck the top of EE's foot causing a contusion. PPE was not a factor, mining systems was not a factor, compliance with rules and regulations was not a factor.
Injured employee was cleaning belt water sprays in a spray bar. They brushed against a fire tap at the location they were working, between the top & bottom belt. When they backed out of the belt and stood up, the fire tap valve suddenly blew off the 8" waterline coupler and struck their left ankle, breaking it. Accident caused by a failure at the coupler tee due to corrosion.
Employee was working on shearer spray arm, when lifting eye that come a long was hooked to broke causing lifting eye to hit their hand. PPE was not a factor, mining systems was not a factor, compliance with rules and regulations was not a factor.
An employee was working on a winch rope on a 650 scoop that was loaded with a shield, this shield was hit by another 650 scoop, causing the employee to be caught between the scoop EE was working on and the shield tip.

Employee was helping use cobra drill when drill was collapsing, they strained their shoulder. PPE was not a factor, mining systems was not a factor, compliance with rules and regulations was not a factor. Employee did not start missing until April 6th.
While mining inby crosscut 23 of the #1 entry in 8 West injured was standing by rib bolting controls when a rock fell out from in between the roof straps and struck them in the back causing a closed fracture of spinous process of thoracic vertebra. No problems with equipment or compliance issues, training or PPE.
