A miner was struck in the ribs with a drill steel that was hidden under the muck in a horizontal position. When the backhoe operator raised EE's outrigger it grabbed the steel causing it to twist up out of the muck striking the miner (right side ribs).

Employee was using a pick to cut a coal sample from the rib. While swinging the pick into the rib, a small piece of coal flew back and hit EE in the mouth, causing a small cut to EE's upper and lower lip. Employee received one stitch in EE's lip. Returned to work EE's next shift.

Injured was helping 2 other miners re-rail a 555 scoop at the 230 foot mark of the slope track. Two airbags were placed to lift the scoop, blocking was being installed to support the scoop to deflate the bags when a crib block shot out from underneath an airbag and struck the injured employees hard hat, resulting in a small contusion to EEs forehead and a sprain/strain to EEs neck.

Employee was installing roof bolts on the right side of the continuous miner. EE had completed drilling a hole and was pushing a roof bolt up in the hole when EE felt a "pop" in EE's right shoulder. No rules or regulations were violated and EE was wearing required PPE. Body failure performing normal job duty.

As they were rolling a rail, EE stopped but employees on the other end continued to roll their end, causing the bar EE was using to swing back and struck EE on the right side of EE's neck and jaw. Failed to maintain control of the hand tool EE was using.

Employee was carrying 20' beams through mud. The employee got EE's foot caught on a cable bolt that was sticking out of the mud, causing EE to fall with the beam in EE's hands. Employee fell to the ground, crushing EE's left hand under the beam and EE's body weight.

Had drilled EE's starter hole and just changed drill steels and when EE started drilling again, a piece of rock fell from the top and bounced off the drill head and struck EE's left forearm. Suffered a contusion with swelling. Missed 3 days of work. Has training and experience and was wearing required PPE.

The rope gripper wear pads on Metz elevator wore down causing a pad wear microswitch to engage that causes the rope grippers to set, resulting in an entrapment of 2 miners approximately 350' down the shaft. The manual rope gripper release pump failed. The miners were removed from the elevator at 5:45 pm. No problems with equipment or compliance issues, training or PPE.

IE was a passenger in a mantrip that was travelling into a track spur. The mantrip struck a dolly that was unseen by the driver, and IE's back struck EE's seat hard. Employee continued to work without restriction until seeking medical attention on 4/24/2026, and began missing days after that. Injury was not the result of a training or compliance issue.

They fell against the side of a roller cradle, smashing right hand between the cradle and roller they were carrying. Suffered a contusion & swelling. Was wearing required PPE including metacarpal gloves.

Injured was cutting down roof screen to unload "bagged" material at 3 block on 3 belt with a grinder and some loose rock that was in the screen fell out causing a laceration to the posterior side of EE's left forearm that required sutures to close. Injured employee was cleaning belt water sprays in a spray bar. They brushed against a fire tap at the location they were working, between the top & bottom belt. When they backed out of the belt and stood up, the fire tap valve suddenly blew off the 8" waterline coupler and struck their left ankle, breaking it. Accident caused by a failure at the coupler tee due to corrosion.
