While helping to hang CM cable and waterline, employee felt a pull and strain in right shoulder. Employee continued to work until Dr. pulled them for surgery on 06/21/2024.

Employee was welding on a chute at the stoker loadout. Employee was in a basket attached to a CAT telehandler. The basket was raised six feet off the ground when it unattached from the telehandler falling to the ground. Employee sustained a fracture to the left tibia.

Employee changed oxygen tank at a permanent station. Attached hose to tank proceeded to open valve on oxygen tank. The regulator failed and blew apart the hose attached to the tank came off as well. Employee was struck by debris from the regulator. Laceration to the left cheek and left orbital fracture.

Carried coats to plant, set them on a belt crossover. Turned to leave area when right foot caught a small pile of coal fines. Employee fell to the ground (same level) onto their right hip and elbow.

Employee approached and entered equipment doors at the slope bottom to exit the mine. Employee closed one door and turned to find the other door near them closing. Employee reached out to catch the door. Another employee was already inside the air-lock opened the out-by set of doors to exit the mine.

EE was unrolling felt liner and picked one end up to go over the dirt pile. EE felt a twinge in left shoulder. EE returned to work but did seek treatment on 01/07/2023. The x-ray showed no fractures or significant findings. Noted and EE returned without restrictions.

Individual was cleaning and raking along MN4 beltline XC96 when they tripped on wire rope falling to the ground. Put their arm out to catch themselves causing a fracture to the Greater Tuberosity.

Helping change ram car batteries. The safety chains were tight, so the car pushed the battery into the rib to take tension off the chains. I.E. reached in to loosen the chains when they became tight again pinching their right index finger between the battery and the safety chain. Caused a fracture and laceration that required stitches to the I.E's right index finger.

Employee was helping another start the face scoop. Employee sat down in the cab and leaned forward to pull on the breaker handle when employee felt a pain in lower back. Through company investigation, information received January 6th, 2023 it was determined this incident need be reported.

2LMN Unit Intersection 10+50 1R XC Actively bolting a brow on a blow through that was 4 to 5 feet in length. The brow broke up causing multiple pieces of shale to fall. Employee was inserting pin into previously drilled hole when employee was struck on the back and legs knocking employee to the ground.

While removing top structure from overland belt the employee dropped the structure pinching right thumb between the structure and structure railing. This resulted in a fracture of the right hand.

While helping build a solid block wall bumped EE's left elbow. There was no visible wound, swelling or discoloration at the time. Reported swelling and inflammation 4/29/2021 and sought medical treatment for cellulitis. Began missing work after Diagnosis of and treatment for infection due to Septic Bursitis 4/30/2021.

While reaching over a ram car to tighten a battery plug, felt a pop below EE's left calf muscle and EE's leg began cramping. Went to Midwest Occupational Health Associates the following morning and was released to unrestricted duty. Became Lost Time after hospitalization 3/17/2021 with pulmonary edema due to a blood clot from the strained area.
