Wm. D. Scepaniak, Inc. · Fall from ladders

While EE was cleaning out the core of the middle cross conveyers, EE was standing halfway up on a 6 foot ladder. While doing this, a rock fell off of the crusher and hit EE in the hard hat. This hit caused EE to fall off the ladder to the ground.

Employee was cleaning the rock piles out of the shoots. Employee was standing on the cross conveyer doing this. Another employee accidentally activated the conveyer, which caused employee to fall from the conveyer to the ground. They landed on their ankle, and this caused the ankle jam/sprain.

EE was removing the screen from the screening plant unit. When EE went to push the screen to the ground, the screen hooked onto EE's sweatshirt and pulled EE off of the catwalk and EE fell to the ground. EE's left leg, left pointer finger, and right elbow were sore from the fall.

EE was clearing out a beaver dam in the area, and ended up scraping the upper right thumb knuckle. EE is not sure what EE scraped it on for sure, but EE did scrape it while clearing out the dam.

When lowing the conveyer, they pulled the pins and it was said that everyone was clear, and this was acknowledged by everyone in the area. However, where EE thought EE's hand was safe was not, and the conveyer was lowered onto EE's left hand. Hand was broken by this.

EE had picked up on the leg of the conveyor with both hands, and removed the pin. When putting it back down, the ledge of the conveyor slipped and pinched tip of EE's middle finger on the left hand. This cut EE's finger open and caused it to bleed.

EE was fixing a wash pump for the wash plant. EE stepped over the hitch of the trailer, and hit their leg on the door of the service truck, which caused EE's leg to be cut. It was the right leg, roughly 3-4 inches above the knee.

EE was moving a water splitter. When EE was pulling it out to move it, the hose bounced back at EE once it was loose. The steel fitting on the end of the hose is what hit EE's forehead and gave EE the cut. Due to the cut, EE then went to the emergency room to see what needed to be done.

Unrolling a conveyer belt. Had to swing it around, and when it unrolled and did a pass it pulled employee's finger in between the pipe and the belt. Got finger stuck in it.
