Showing all 6EE stated that EE was oper Volvo A 35 Haul Truck when the seat frame broke causing the back of the seat to fall back. The seat was fixed on Saturday and put back in service. EE called on Monday 9-18-2023 complaining of pain to EE's back
EMPLOYEE WAS HELPING STRING A NEW BELT ON THE SREENING CONVEYOR. EE HAD LEFT HAND ON THE RAIL OF THE CONVEYOR. THE CHAIN THEY WERE USING FLIFFED OFF THE 4X4 BAR AND HIT EE'S HAND. EE RECEIVED A BROKEN INDEX FINGER ON LEFT HAND FROM THE CHAIN AND 4X4 WOOD BLOCK. EE WAS TREATED AND RELEASED FROM HOSP.
Operator failed to put haul truck in park and take transmission out of drive. Operator also failed to chock wheels after exiting the haul truck. The operator then attempted to jumper the haul truck with jumper cables. The operator was standing between the the two trucks and when the battery was jumped it caused the first haul truck to move pinching operator between the two trucks
Miner was reinstalling a drive belt when hand slipped and right hand hit the conveyor frame. The conveyor was locked out of service and not running. The conveyor was manufactured in house.
EE was removing a wedged rock in a v-belt. The rock was removed and let the pulley move forward pulling EE's right ring finger into the pulley. The tip of the right ring finger was broken and cut. EE was taken to a hospital where the finger tip was reattached.
