Employee was using a bar to dig out rock that was stuck in a cone crusher. When the rock broke, the bar hit the mantle, pushing the bar back and turning it in the employee's hand which cut employees fingers.

Defective Radiator cap blew off water pump resulting with Burns on employees right side (Arm, armpit, back, stomach area).
Hearing test showed a standard threshold shift in left ear.
Employee was performing maintenance. Employee was removing rock from the jaw crusher. They put a rock back into the feeder and the rock slipped from their hand causing their hand to be pinched between two rocks.
Employee was moving rock on the back side of the rock box of the jaw crusher. EE's left pinky finger was caught between rock and the rock box. Laceration on outside of left pinky.
Employee was removing flip gates from bypass chute. Right thumb was pinched between the side liner and flip gate.
