Loader Operator was operating a 980 CAT Wheel Loader. While driving through the facility in the Loader, the Operator drove over an uneven road causing the air suspension in the Driver Seat to lower. As the seat lowered to full extension, the Operator felt pain in EE's lower back.

Showing all 4Employee was stepping down off the ladder of the haul truck when EE's right foot planted, EE went to step forward and the right foot slipped in the mud. EE grabbed for the hand rail and felt a tear and pull in upper arm.
Employee was tied off and working on loadout spout out of a manlift, when a customer truck disregarded stop lights and entered the scale. Upon entering scale, truck's cab sleeper came in contact with manlift boom and made it sway, this caused EE to lose footing and fall to the floor of manlift which caused EE to strain back.
Employee was moving a 6" hose. When EE let go of it, EE's finger was some how pinched causing a cut on EE's right middle finger. EE was taken to walk in clinic to receive first aid. They cleaned and dressed the wound and released employee.
