On 3/17/26, the employee stepped on a rock and overextended the arch of their right foot. The injury was initially believed to be minor, but the pain persisted and worsened. On 4/13/26, the employee was evaluated at a clinic and prescribed an orthopedic boot. X-rays were negative for fractures; an MRI has been ordered for further evaluation.

While performing maintenance, an employee was manually removing a section of metal floor grating. The grating shifted unexpectedly, catching and pinching the employee's right middle finger between the grating and the support frame. This resulted in a laceration to the fingertip which required sutures for closure.

While performing a drilling task, the operator was applying manual pressure to the tool. The drill bit snapped unexpectedly under tension. The sudden release of resistance caused the operator's hand to lunge forward, striking the jagged edge of the broken bit and resulting in a puncture wound to the right index finger.

Miner had completed sounding, mechanical and hand scaling of the back and ribs and was preparing to begin the bolting process. As the miner turned towards the machine a piece of rock bounced off their hard hat and shoulder before falling and contacting their left ankle resulting in a compound fracture and a fractured neck vertebrae.

Miner was contacted on the right lower torso/rib area with the airline spud and wingnut resulting in fracture to the ribs. The airline was caught on the cab of the mucker and was not seen by the miner until it had pulled tight and released striking the miner.

Miner was cleaning rocks out of the apron feeder in the tunnel. The left index finger was smashed between the bar the miner was using and the apron feeder wall. The contact resulted in the tip of the left index finger being fractured.

Employee was operating Jackleg when the gooseneck came apart and struck the employees left elbow resulting in a contusion.

On 2/28/25, an employee injured EE's right knee while unloading drums from a trailer. EE climbed over drums to access tie-downs and jumped onto the trailer bed, landing awkwardly. Initial care was conservative. PT was later tried for symptom relief. In July, surgery was required, and the employee is now off work recovering.

The employee was using a hook knife to cut zip ties from a band when the knife slipped lacerating the tip of the middle finger on the left hand.

Miner had completed drilling hole in ore face, their right hand slipped from the D handle as the hammer cammed over and pinched right middle finger between the hammer and the leg connection point.

EE was assisting with connecting air compressor to p/u truck. EE attempted to move the tongue of the air compressor to the hitch of the p/u truck when EE felt a snap in their left shoulder.

Employee picked up a flex hose to move it. During the movement something flew into EE' eyes. Later during the shift EE' eyes became itchy and irritated. The employee was wearing safety glasses at the time of the incident. The employee was taken to the Emergency Room where EE was treated for a bilateral abrasion of the cornea. EE was given an antibiotic to treat the scratches.

Miner was aligning steel frame for air door installation. Miner asked for a spud wrench to align the top and bottom steel frames. During this time the steel shifted and the tip on the Miners left index finger was pinched in the hole resulting in a crushing injury with lacerations.

While stripping wire insulation with a razor knife, an employee's grip slipped, causing the blade to cut their left index finger on the opposite hand, resulting in a laceration.

Employee received a laceration to the tip of the index finger on the left hand while operating a drill press. This required 5 stitches to close the wound.

While training a miner using a jackleg with a 4' steel to drill a bolt hole in the back, EE was advised to reposition the leg. While repositioning the leg, the miner gave the cob too much pressure losing control of the drill. The drill fell backward towards the trainer hitting EE in the face splitting EE's lip. *The miner received 3 stitches by the medical facility's doctor.

Employee installed Leach Tank #3 lid section to fit into place. The lid was stuck causing employee to push lid with right leg causing overexertion of their knee. Employee did not notice any pain at that point in time. They reported the incident on 7-10-2023. Doctor recommended Physical therapy on 07/18/2023 to have done. Physical therapy will start on 7-21-2023 @ 4:00 PM.
