February 27, 2026, an employee and a coworker were cleaning tracks on excavator. While holding the hose of a sprayer as the coworker operated the sprayer, the employee slipped from the track surface and fell from the equipment. During the fall, the employee's right leg became caught between the excavator tracks, and the employee fell to the ground.

A haul truck operator was pulling away after being loaded when an excavator bucket contacted the truck tailgate. The excavator operator believed the truck bed might roll over and attempted to stop it with the bucket. The contact jolted the truck operator in the seat, causing lower back pain. The operator was transported to a clinic and diagnosed with a lower back strain.

While clearing frozen sand rocks from the Alpha hopper, employee was stepping down to the ground and slipped on their final step. They were able to catch themselves and did not fall; however, during the slip they sustained a cut to their left leg from contact with a metal edge.

IE was cleaning a silo spill. IE was pushing a pile of sand through the scale. At the end of the scale, the scale meets concrete and there is a small rise. The bucket clipped the concrete causing the IE's impact on the windshield. IE had a hard hat on; the brim caused the gash.

The crew was using a sky track with a man basket in an area where a conventional manlift could not be used due to ground conditions. While positioning the boom, a pinch point existed between the man basket and sky track backplate rack. The employee was unaware, and as the boom was raised, the basket shifted, catching and injuring fingers, requiring five sutures.

On February 3rd, 2025, at 4:30 AM, an employee pulled back a hose with excessive force, lost balance, and rolled their left ankle while trying to stabilize. A crunching and popping sound was heard. Unable to bear weight, the employee's foot was dangling. Work partners, the foreman, and safety were notified. The injury was classified as recordable on March 2nd, 2025.

Miner was using an impact wrench to remove the bolts from the tail bearing. When they removed the last bolt, the bearing jumped up, trapping a hand between the bearing and the take-up guard.

An employee was assisting in replacing slurry pump PU6. While hoisting the new pump into place with a telehandler and chain hoist, it became stuck. The employee attempted to reposition it by placing EE's right-hand underneath. When the bind released, the pump shifted, crushing EE's right ring finger between the pump and the skid.

Employee was utilizing a grinder to cut a bearing sleeve. This task was necessary to replace the bearing on the head pulley of the C1 belt. Midway through the task, the grinder unexpectedly kicked back towards the employee, causing the grinder's disk to make contact with the employee's left forearm resulting in a laceration.

EE was inside the office section of the warehouse when EE encountered a bee and was stung on the left side of the upper chest area. Stinger was removed, first aid sting ointment was applied, drew boundary around wound to monitor. After several minutes, there is a small amount of swelling, but employee appears to show no signs anaphylaxis.

Employee was cleaning EE's area after finishing a project. Employee was changing air bladders on a screener. Employee bent down to pick up the bladder, EE pulled on it. The bladder was caught on the grating, it released suddenly and the employee fell back against the handrail and struck the left side of EE's body.

Injured employee (IE) attempted to provide maintenance on the PU 6 pump belt. As other employee was performing that task, IE removed the keeper that was in place and opened the guard gate to inspect the tension of the belts. The motor immediately began rotating as IE placed hand on the belt. This caused IE's left hand to pull into the belt, causing injury to IE's fingers.

Employee was helping change metal screens on a T190 McCloskey. Employee was trying to fit a U-bolt in place, to hold the screen in place. (Bottom deck Screen tensioner) The U-bolt plate slipped down smashing employee's finger between the screen plate and the U-bolt.

The bee had traveled into the employee's sleeve while outside, then the employee thought it was fly and tried swatting it out of the sleeve and ended up getting stung twice.

While connecting conveyor sections, section DF contacted section DE causing section to slide off the dunnage it was resting on. Employee's foot was pinned. The movement continued to slide across the steel toed portion and came to rest on employee's foot, causing injury. Employee was given first aid on site and transported to ER where employee was treated and released.

At 7:53pm on 4-26-23 employee was using Skid Steer #12068 cleaning a sand spill in silo 2. When the blade from the bucket of the skid steer came into contact with the seam from the scale causing the skid steer to stop abruptly thrusting employee forward. The act caused a discomfort in neck and shoulders.
