A water truck air hose burst & the brakes locked on while going down a dirt road, incline 5.35%. The driver thought the brakes weren't working. EE tried the foot pedal & air actuator & gearing down. Once in neutral EE could not get it back in gear. EE steered the truck off road and jumped while going around 15-20 mph. Driver strained EE's hamstring.

Showing all 7Two workers were attempting to get a coupler off a plastic pipe. One worker's hands slipped as they tried to turn the coupler and their left ring finger hit a pipeline component or the pipe wrench causing two cuts to this finger. Sutures were applied.
On 9/24/24 EE reported hand was hurting. Right palm under EE's middle finger looking faintly bruised. EE stated was not positive but thought it was from using the impact wrench working on the tails feed system. On 9/24/24 EE asked to go into the doctor for an evaluation. We did not receive a release until end of day 9/25/24 stating Ee needed to be on light duty.
While preparing a cathode sample for analysis using the lathe when a piece of copper flew off and cut the technicians lip. Tech was taken to the hospital and received one suture to close the cut up.
Employee was washing down under CCD's. Employee was spraying down material to the sump area and was standing on a pipe when employee slipped off and twisted left knee. Employee went home for the remainder of that day and the following due to the knee being sore.
Employee had parked the forklift and exited the forklift. EE started walking down a slight incline and slipped on ice causing EE to do a front/back split with EE's legs. this caused a strain to EE's right groin area, creating a lot of pain.
Employee was cleaning off a 25' tape measure & trying to get it to roll back up. The tape measure was getting stuck when rolling back up. The tape measure suddenly started to roll up & cut vertically across the top pad of the right thumb. EE cleaned & bandaged their thumb & went back to work. Later they went to the ER received 5 sutures.
