Employee left remote on box, went to make final adjustments with the remote, and when EE hit the down control, it smashed EE's left pointer finger between the frame and the remote. Stiches required to pointer finger.

Small amount of material was noticed inside the belt and tried to remove it with their crescent wrench, when the wrench became caught in the tail pulley, the employee's left arm was removed as a result of the entanglement within the tail pulley.
